While sweeping the garage floor, the broom handle the Miner was using broke causing them to cut a finger which required 3 stitches to close.
While operating the telescopic forklift, the miner opened the left-hand door, high wind pulled the door from the miner's grip, broke safety strap, the door hit the frame and shattered the glass creating flying debris. Debris was removed from beneath the eyelids, an ointment applied, the mine returned to work.

The Miner was found unresponsive in front of their parked Caterpillar 777D Haul truck. They reported that they don't remember anything after descending the upper ladder of the truck.
While installing a pin in the bucket cylinder of the PC2000, the Miner had their foot positioned under a pallet board. The pin came loose from its rigging and struck the pallet which in turn struck the Miner's left foot behind the steel toe shoe.
After filling the radiator with fluid on the Epiroc D65 Drill and returning the tools to the grease truck, the Miner returned to replace the radiator cap. Mud on Miner's boots caused the Miner's feet to slip and fall from the ladder landing on right leg, spraining knee.
While walking from Miner's Epiroc d-65 drill to talk to the operator of another drill, the Miner got dust in right eye which caused Miner to rub eye and dislodged contact. Miner's eye became increasingly irritated and Miner sought medical attention on the second day. Miner was diagnosed with a scratched eye and has been treating for the same.

While adjusting the arm of a new pair of safety glasses, the glasses slipped beneath the Miner's thumb while applying down pressure causing the opposing hand to swing up and towards the Miner's face while holding the glasses. The arm of the glasses struck the Miner's left eye causing a tear.
While repairing a bent mud flap bracket on the HM400 Articulated Truck, the Miner attempted to cut the rubber mud flap away from the tire. The utility knife that the Miner was using slipped and struck the right pointer finger causing a cut that required three stitches to close.

While moving pieces of drill steel with the Forklift, one of the pieces of steel slid off the side of the forks. The Miner manually slid the steel to remove it and the steel caught the pocket of Miner's coat causing Miner to be dragged to the ground. The Miner struck head on the forks during the fall.

While making rounds in the company site truck, the Foreman struck a rock in the dark with the front tire that caused the steering wheel to snap around. This resulted in a strain to left wrist and forearm and was diagnosed with torn ligaments.
While removing the oil drain plug from the rear axle of a Caterpillar 777D haul truck with a breaker bar and socket, the Miner struck EE's left hand on the trucks strut bracket when the plug broke free. The Miner cut EE's middle finger below the knuckle on the bracket. The Miner was taken to the hospital where EE received stitches to close the laceration.
While parked on level ground the Miner dismounted the 777D haul truck to take a break. When stepping down to the ground from the last rung of the access ladder, the Miner felt pain in upper right leg and was unable to walk without pain after completing the step.
After observing the operation from a vantage point adjacent to the northern light plant, the Foreman turned and proceeded toward a vehicle that was to the rear and facing them with the lights on. As they stepped down with the right foot approx. 1' to a different level, encountered a small patch of snow-covered ice causing them to fall on right-rear side.

While bending over to cut the strapping on a pallet, the Miner's footing slipped causing EE to stab self in the leg with a pocket knife.
While aligning the track linkage on a Komatsu D375 dozer with a pry bar as the pin was being pressed out with a hydraulic press, the Miner's hand was punctured by the pry bar when the pin released.
While operating the Caterpillar 777B Haul Truck, the Miner jarred EE's back when the truck struck a dip in the road.
While laying out the drill pattern, the Miner slipped on mud causing Miner to fall onto hand and break middle and ring finger.
While cutting a block of metal from a sheet of steel, the block of steel was levered by a support block once it became free. The block struck the Miner's helmet and then the Miner's forehead which caused a cut on the forehead.

While operating the 777B haul truck, the operator jarred EE's back in the seat
While installing a deep well pump, claimant fell into an adjacent ditch and struck leg on a rock causing a laceration which later became infected.
While backing up to the dump, the miner backed through the berm causing the Caterpillar 777B haul truck to roll over onto the embankment, coming to rest on its canopy and nose. The miner was wearing seat belt, but received facial injuries during the rollover.
While replacing a cooler on the Atlas Copco D65 drill, the Miner was underneath the drill securing the bottom bolts. Dirt fell from the machine and struck the Miner in the face resulting in a a paint chip in eye.
While striking a metal bucket shroud to remove it with a hammer, a sliver of metal came off the hammer and embedded into the welder's left leg.

While dismounting the Komatsu D375 dozer, the Miner slipped from the push arm causing them to fall and break a bone in their hand on the ground.
While operating a Komatsu HD785 haul truck during loading operations, a large rock fell from the Komatsu PC3000 backhoe that was loading the truck. The rock struck the tail end of the truck bed causing the front of the truck to bounce on the ground. The miner jammed leg causing discomfort to back and leg.
While helping the welder spread out a tarp for protection from the rain, Miner was splashed in the face causing a small metal shaving to get into the left eye. Miner did not immediately seek medical attention but later had it removed by a specialist.

EE was repairing parking brake on haul truck. His hand was on drive shaft when operator accidentally put truck in gear. EE's hand was pulled into drive shaft which jerked his entire right arm in a circular motion. EE refused first aid and drove himself to local ER He was treated and released.
