Employee stated EE was walking back to the section when EE rolled EE's ankle. Employee sought medical attention and was taken off work.
Employee stated EE was walking to the shop and slipped and fell in mud located at #9 bottom. Employee was taken to the hospital and was diagnosed with a broken ankle. This incident is being reported as an LTA.

Employee stated EE was hosing out the bottom of the plant when EE stepped into a hole and fell backwards landing on EE's right shoulder. Employee sought medical attention and was taken off work on 5/1/2023. This incident is being reported as a LTA.
Employee was walking and felt a pop in left knee. Employee started missing work on 3/1/2023.
Employee slipped and fell in an area leading to a sampling point. When they fell they hit their head on a metal beam discarded in the area. The impact of the fall caused a cut on their ear. There was a mat in the area that caused the employee to slip when they stepped on it. The area has been cleared and the owner has put down gravel making for a safer path of travel to the sample
Employee stated they were exiting the #10 Volvo truck when they slipped and hit their right knee on the step. (tore meniscus)

Employee was stepping over the panline on the Longwall face when employee stepped into a hole and strained left hamstring. The employee has continued to work but has now started Physical Therapy on 9/1/2021
Employee tripped while going thru a mandoor and fell striking EE's forehead just above the right eye on the door latch. Employee suffered a laceration that required 10 sutures.
Employee was burning on both sides of the belt line when employee stepped in a rut and felt a pop in right knee. The employee continues to work but has started Physical Therapy on 06/25/2021.
Employee tripped over a rock dust hose on the catwalk at #1 sump. As employee attempted to catch self employee strained both shoulders. The employee continued to work but started Physical Therapy on 6/28/2021
EE was dismounting the locomotive at prep Plant load out. As they came down the steps they slipped and hit left forearm on the steps. EE worked till 2/23/2021 when taken off work.
