A roof fall was discovered at break 10, #2 entry of 78 HG panel. The size of the fall was approximately 12ft H x 16ft W x 20ft L.
Employee stated while trying to get a piece of stuck drill steel out of the roof the steel fell out striking the employee on the left ring finger causing a laceration and fracture.

The #9 elevator door switch failed
The employee was manually operating a GOAB switch when an overhead porcelain insulator broke into multiple small pieces. One piece fell and knocked off the employee's hard hat. A second piece fell and made contact with the right side of EE's head, near the hairline, resulting in a laceration that required medical glue for treatment.
The employee was shoveling along the 79 Mains North belt when EE's left thumb came in contact with a bottom roller, causing a laceration. The employee sought medical attention and was given eight sutures and taken off work. This is being reported as an LTA

The operator had to tram forward to clear a catwalk, and one wheel of the man-lift sunk in soft material, which caused the lift to tilt. The lift shut off automatically, and the employees were stuck until lowered by the FD.
The employee was in the bolter station when the miner advanced a piece of rock that got caught between the back support and the rib, causing the jack to yield due to the bolter back stop not fully extended, causing the employee to sprain their left ankle. The IE felt a piece of coal/stone hit L.hand from an elevation while hoisting a new siding panel with a rope & pulley. Prior to removing the old siding, the floors were cleaned near the panels to be removed. Not certain how/from where it fell. The whole bldg vibrates and there is a lot of area. Non-displaced transverse fracture of the 1st metacarpal. Restricted duty 4-6 weeks.
