While monitoring a slurry tank pump due to abnormal noise and visible smoke, the employee was positioned in the area when material was unexpectedly expelled from the pump. The employee stepped backward and lost balance, resulting in contact with the ground. The employee reported an abrasion above the right eye and discomfort to the right hip.

An employee was using a break-over binder and chain to secure poly pipe to a float, the chain slipped, causing employee to fall awkwardly, (same Level) breaking EE's lower left leg.

12/7/23 Reported twisted right knee at Dry Plant while walking in 3" of sand. Medical examination diagnosis sprain, still in pain. 12/12/23 Requested MRI. 03/18/24 Received Diagnosis that surgery required to repair knee. Surgery TBD
