EE was walking in GEHO Sump area #1 when left foot slipped on a small rock causing EE to lose their balance and putting full weight on the left foot on the ground surface. This cause a left Achilles tendon rupture.

Employee threw the cable towards the back of the refuge chamber and stepped down from the first rung of the ladder. Employee slipped as employee stepped down and felt a sharp pain in right ankle and fell to the ground.
