Struck by falling object

Drill helper removed stuffing box, tube lowered to safety block, pulled overshot off tube, stepped back. The driller ran rods up to release tube from rods. Once the driller ran the rod string up, a piece of slipped core (or core stuck in bit/shoe) fell out of the rod string as rods moved forward, striking the helper in the shin.

At approximately 3:00pm the service cage was downed due to the brakes being applied and not releasing during a pm of the conveyance hoist. A relief valve was not releasing pressure to release the brakes.

At approximately 8:25am, the supervisor was notified that an a ground fall had occurred in an inactive drift near the face. The wire was hanging down and SX ground support bolts were attached to the wire.

Auxiliary Cage (MaryAnn Cage) was shut down at approximately 6:00 pm on October 30, 2025 due to accumulator leak of the hydraulic power unit. Accumulator was slowly leaking off nitrogen charge.

EE was walking in GEHO Sump area #1 when left foot slipped on a small rock causing EE to lose their balance and putting full weight on the left foot on the ground surface. This cause a left Achilles tendon rupture.

At 5:02 p.m., the hoistman was raising the Service Hoist to the surface with one miner on the cage. The Shaft Crew had just finished installing a new remix on the bottom of the slickline, approximately 650 feet from the surface. The hoist tripped out due to the electrical drive overheating.

Around 9:30 am an Employee was loading a UTV on the service cage at the surface collar and the overhead door came down catching the buggy bending the door pulling it out of the track. The cage is used for either personnel or equipment and at no point were persons endangered. Operator is filing this report under duress.

Rubbed or abraded

Employee was drilling vertical holes above head with a Hilti gun using a 1 ½"" bit to install anchor bolts to secure a 10' pipe. Employee was taking a break and wiped their face off with a rag, and got a piece foreign object in their right eye. EE then attempted to flush it out. Foreign object was later removed by optometrists.

Employee was at the 3090-refuge chamber pulling the 3/C #10 AWG cable around to the back of the refuge chamber to land the cable into the junction box. Employee threw the cable towards the back of the refuge chamber and stepped down from the first rung of the ladder. Employee slipped as employee stepped down and felt a sharp pain in right ankle and fell to the ground.

While Hoist Worker was performing daily checks on Auxiliary (Mary-Ann) hoist Hoist Worker could not get the hoist to function. The electrician on shift was called to troubleshoot the problem. Electrician trouble shot throughout the day while being in contact with the manufacture to assist in troubleshooting. Parts will need to be ordered as the issue in in the internal drive.

Employee was chiseling out dried and hardened cement inside the Paste Plant Mixer Hopper. While attempting to chip out a section of the hardened cement with an air chisel using the right hand, EE had their left hand behind the hardened cement. When the air chisel broke through the cement it contacted EE's left ring finger causing injury to that finger.

Operator was using a 6' pry bar to free 5"" steel balls inside of discharge kibble bin. While attempting to the pry under the 5"" steel balls, the balls broke free and forced the pry bar to move in the direction of the EE's side of the right face striking EE on that side of the face.

Employee was cleaning material buildup on the grizzly screen of a feed hopper. When attempting to step off of the grizzly bars EE's right leg fell through the grizzly bar openings (about 6"" opening), causing injury to EE's right knee after hitting the metal bars.

When the oncoming shift was headed to the 2950 4503 to retrieve their equipment it was noticed that material had come down from the back. This is approximately 37 feet from the intersection and the fall is approximately 25 feet in length and 4 feet in Hight and 8 feet wide.

Struck by flying object

Injured worker was grinding on metal in the shop. They did not feel any pain but when they arrived home, they noticed what appeared to be a small piece of metal in their eye.

Over-exertion in wielding or throwing objects

Injured worker was using an air lance at the paste plant to clean a hopper and felt tightness in their back after the shift. Employee reported the following morning that they still felt tightness in their back and needed to seek medical attention.

Injured worker was retrieving a tool from under a haul truck. When injured worker stood up their knee gave out.

The auxiliary hoist went down at 12:30pm after making a trip underground. When the cage was called to surface the auxiliary hoist drive faulted.

A power bump occurred across the site and caused damage to the cage proximity sensors at the shaft station. Hoist was not able to operate in automatic mode and the operator had difficulty transitioning it to manual.

During use the materials hoist stopped due to the brakes locking to the hoist drum. After investigation it was found that a failed brake control sensor caused the brakes to engage.

While fitting drill rod wipes to the drill pipe, driller moved head causing top section of drill pipe to fall 2"" resulting in a pinch/crush injury to employee's right pinkie finger. This resulted in the amputation of pinkie finger.

Employee was using a pneumatic blowpipe to clean out accumulated dry tailings in a conveyor transfer chute. While inserting the blowpipe into access hatch the air control valve struck the side causing a significant increase in airflow. The blowpipe pushed back against the employee's grip causing their shoulder to extend.

Employee was walking down a flight of stairs from the Conditioning Tank down to Cleaner #1 in the mill when they slipped/tripped on a step causing injury to their left leg (knee, shin, and foot).

Employee was offloading materials in the service cage at the shaft collar. Forklift made contact with the door causing it to dislodge from track. Unable to fix within 30 minutes. Damage didn't interfere with the use of the equipment which is designated for materials. Operator is filing this under duress due to the ambiguous regulatory interpretation by District leadership.

IP was using a hammer to strike rods to loosen stuck joints when IP lost grip and control of the hammer causing it to bounce back.

While loading materials onto the service cage, contact was made with the door causing damage. The cage is used for either personnel or equipment and at no point were persons endangered or was the equipment damaged which interfered with its use. Operator is filing this report under duress.

Contact with heat

Driller stopped work after identifying the driller helper began showing signs and symptoms of heat-related illness. As they were walking to the cool-down station the drill helper's symptoms escalated. So employee was taken to surface for eval and taken to local ER for evaluation, treatment and returned to work.

Over-exertion in pulling or pushing objects

IP was using chain hoist to raise sump pump when the hoist stopped. IP jerked on the chain and felt a pain in shoulder. IP continued to work the remainder of the shift and the following day. Injury was reported to operator on 5/17/23 and accepted by WC on 5/30/23.

Fall to the walkway or working surface

A process operator was pushing material with a dozer at the tailings stack when they exited their equipment to clear off accumulated rocks/materials from the track undercarriage. The employee turned to re-enter the dozer and tripped over a rock. The employee extended their left hand during the fall, taking the full weight of the impact onto their hand wrist.

An employee was struck by falling material while loading a face round in the 2896 heading. The employee (E1) was holding the pneumatic ANFO delivery hose and turned toward the face while another employee (E2) was operating the loading controls from the basket of the powder truck. E2 witnessed a falling rock strike E1 in the left shoulder and upper back.

Two employees were tasked with applying foam grout to the 2940 heading. While on a scissor deck a standpipe was sticking out and obstructing the deck. One of the employees found a ratchet tie-down strap and attempted to use it to pull the standpipe out of the way. While using the strap, the hook ends slipped and rebounded toward the employee, striking them on the left shoulder.

Worker was on the Bench/Face of the COB #2 cleaning out drilled holes to prepare for loading of explosives. Worker was using a copper blow pipe with a 90 deg bend in it. While cleaning the hole, a piece of rock struck the worker in the face near eye brow of the left eye. Laceration to forehead, 3 stitches.

Fuel delivery line in exhaust shaft became damaged due to unknown reason and separated. Fuel spilled during transfer to underground. Approximately 380 gallons discharged into mine dewatering system.

While dumping in the south scrolls the #2 skip became lodged. The hoist continued to move, attempting to lower the skip from the dump position and spooled out 365 feet of rope before the equipment was stopped.

While performing maintenance on a de-energized pump employee attempted to rotate the drive belt between the pump and electric motor by hand which resulted in right ring finger being pinched and lacerated between the belt and pulley. Employee received 5 sutures on 7/29/2022.

Over-exertion in pulling or pushing objects

Employee was pulling weeds around the property on 6/18/22 having some lower left back pain. Resumed on 6/19/22, pain radiated to the left thigh and back. Taken to the ER. NSAID given. 6/20/22 returned to work and complained that the pain was still persistent. Taken to the ER. Given pain medication and 3 days off of work.

While rolling up the water hose, the hose came off track of the hose rail, stopped rolling hose. Corrected hose and continued to roll up water hose and right ring finger was pinched between hose rail and inside support.

Employee was using a table saw to cut plywood. Employee's flinger came into contact with the saw, resulting in laceration.

Employee was removing a press cloth from press 2 on the second floor of the press house that was full of mud. The cloth released during movement of the cloth and the force being used was suddenly no longer in the range of motion and the employee pulled EE's shoulder.

Got into the bed of work pickup truck to retrieve an item from the in-bed toolbox. When exiting pickup bed, grabbed onto the tailgate with hands. Lifted left leg over tailgate and onto rear bumper. Was lifting other leg over tailgate when fell to the ground on back with right ankle bent backwards. Not sure if foot slipped off bumper or what.

Individual was removing kelly rod from rod rack when hose that was attached to water swivel contacted individual in lower lip. This resulted in laceration to the lower lip.

Individual was in the process of pulling core tubes. Individual finished shift; however, cumulative duration resulted in aggravation of the individuals back.

Struck by flying object

A mechanic was installing a bushing on the z bar of a scoop with a push plate by hitting it with a sledge hammer. While hitting the push plate with the sledge hammer a piece of the push plate flew off. The shrapnel flew and cut through the employees glove and lacerated the right hand knuckle.

Material was being mucked out of the left side of the face area. During material removal rock bolts were found in the muck pile from strata weakness in the left side of the heading. 2798 Ramp 02

Employee was carrying three 70 lbs jumbo drill steel from lay down to shaft on surface. The weight of the steel became too much to handle and employee tried to set the steel down. While bending forward employee's hand contacted the ground and the steel hit the top of employee's right hand causing a fracture.

The injured miner and two others were attempting to retrieve parts from a dismantled Galloway. The galloway was located at the 3000 vent shaft bottom. The three miners were lifting a metal sheave wheel from the galloway when it slipped from their hands pinching the right little finger of the injured between the sheave wheel and other metal.

There was an area that was recognized to have a falling hazard. During the attempt to remove the hazard. Injured Employee was walking into the area and was struck with falling material.

Employee was getting ready to weld on the ball mill feed chute when the dog being used to push the plate tighter to the chute popped off and struck the employee between the eyes causing a laceration. The employee was wearing the proper PPE.

Employee injured left thumb while opening the rear axle cover on the Cat 1600 LHD. As they lifted the hood with right hand, left hand rested on top of the cab and as the cover passed the cab edge it pinched left thumb between the cover and the cab. This caused a laceration leading to the prescription of antibiotics.

When checking the upper limits switch on the service hoist, the hoist operator brought the service cage up into the headframe without de-clutching causing the counter weight on the other side to drop out of the guides. No one was in the cage, this was during pre-shift checks.

Struck against stationary object

Employee was operating a stand behind dingo loader. Employee started to raise the bucket to tram the loader away from the area when the bucket stuck in the raise position, the bucket was overloaded and the machine tipped forward causing the operator to be pitched over the front of the machine causing a dislocation of right knee.

Employee was walking to work area lost EE's footing and slipped on loose gravel. EE's immediate reaction was to reach and grab for a wire running along the Rib. In the process, EE dislocated EE's right shoulder.

Struck against stationary object

The employee was installing steel in the main shaft. EE was using a hammer to pound the steel into place. The back of EE's hand made contact with a metal wall bracket causing a laceration that required stitches. The employee was wearing gloves.

Over-exertion in pulling or pushing objects

Employee was installing 10 inch pipe in the shaft, the pipe shifted away from them the employee tried to hold the pipe in place and felt a pain in left shoulder. Employee worked at full capacity until surgery on the 28th of September 2020.

An employee got rocks and debris in the right eye while cleaning a down hole blast hole.

DRAEO is providing short term consulting services at the client's operation. The injured employee was asked by a client employee to assist in removing a rock from a locked out conveyor. When setting the rock down, the injured employee's fingers were caught between the rock and the conveyor structure, ultimately resulting in the injury.

Blasting occurred at the 2900-493 geo-technical test stope causing ground fall to go past the ground support of the back of the top cut of the geo-technical stope.

Employee stepped foward with left foot, pushing off with right foot. Employee heard a loud pop. The 4th and 5th toe of right foot are broken. The ground the employee was on was uneven.

The East North Vent Shaft Rope was damaged when it was pinched between the VSM bucket and the shaft form while attempting to remove an oversized rock from the sinking bucket. The rope was thoroughly examined at 5:20 am and the hoist was taken out of service pending repair to the rope.

The East Main North Vent shaft hoist went down due to Electrical issues with the PLC communications. The hoist was repaired and back in service at 2:10 PM.

The East Main Shaft hoist power tripped due to a failed 24-volt DC power supply. The replacement unit was installed and the hoist was put back into service at 10:30 am.

The East Main Hoist drive failed causing the hoist to be out of service. The hoist was repaired and put back into service.

An employee was pulling on the Jackleg extension to move it closer to the hole being drilled when EE felt pain in EE's right arm. The employee suffered an injury to the tendon near the elbow.

Fall from machine

EE sprains knee when EE felt knee pop while dismounting water truck. EE transferred weight from one foot to the other on factory truck steps and lost balance, falling to the ground.

The employee received a laceration on left hand when it was hit by a rock while scaling. The laceration required two stitches to close.

The injured employee was hit on the foot between the steel toe and the metatarsal guard by a rock that fell while loading a face round. The injured sustained a laceration and fracture of the left 4th toe.

Contact with electrical current

A welder received a shock while welding a phone enclosure on the top deck of the Galloway. The employee was transported to the ER and was released to light duty.

Fall onto or against objects

During bolting activities the injured miner was attempting to get out of the way of a rock and was either struck by the drill of another miner or fell into bolting supplies that were leaning against the shaft wall. The miner suffered a lacerated nose and scraped elbow.

The East Main Hoist shut down during night shift due to a heat sink fault on slave drive #1. The 120V to 24V step down transformer that supplies power to the drive cabinet failed. This caused the hoist to trip on heat sink.

The employee was working on the surface at the east north vent shaft discharge water line to fix joints in the pipe. The Employee strained their lower back while using the pipe jack to clamp together discharge water pipes. The Employee was in a crouched position when twisting and turning the jack handle.

On 11-20-13 operating at shaft sinking operations at mine. Mine experienced a power outage coming from the sub station in Yerington, NV. Troubleshooting problem & determining power may be our for awhile we notified MSHA @1303 (EE & another EE were currently conducting a inspection at the time) and removed the miners via Grove RT890 crane and a manbasket.

On 8-19-13 while operating a shaft sinking operation at the Pumpkin Hollow Mine. At 7:25PM the hoist experienced a loss of power due to an electrical storm. Miners were evacuated using the Grove 890rt crane with manbasket. After the electrical dept. trouble shot & reset the hoist. The hoist resumed normal operations at 9:30 PM. MSHA was notified via the 1-800 Number at at 8:05 PM.

On 8/6/13 while operating a shaft sinking operator at the mine the hoist experienced a fault on over and under wind of the hoist control panel. The fault would not let the hoistman operate the hoist The electrical dept. trouble shot the fault reset and tested the hoist. The miners in the shaft exited the mine via secondary escape using the 890 RT crane and man basket.

On 6-07-13 the North side chute had an issue causing operations to be put on stand by at 12:35 am. Trouble shooting found the N. chute trunion pin flipped. Operations were halted at 1:10 am and as a precaution MSHA was notified that hoisting would be stopped and the miners would be extracted. All were out in twenty minutes. No miners were trapped or injured.

The Hoist operator experienced a brake wear fault on Hoist. The brake wear indicator failed. The linear potentiometer is located and secured on the rear arm that is attached to the counter weight. It cycles on a hydraulic style cylinder which tells the hoist of the brakes need replaced.

EE was reaming holes with a magnetic drill press with a 1-5/16"" drill bit. The EE stuck his gloved hand near the bit to clean the cuttings away and glove became entangled lacerating right index finger and severing tendon.
