While opening a drawer in a tool box mounted on the catwalk, employee stepped backward and put foot under guard rail. He fell against the handrail striking his back and ribs, fracturing a rib.

At the time of the incident EE was climbing down the boarding ladder of a water truck. When He reached the bottom of the ladder he extended his right toward the ground and hopped off the ladder.Upon landing his right knee twisted and he fell to the ground.

He was crossing over a mobile bridge system and tripped and fell. He continued to work and started missing work on 8/30/05. The original doctor's report showed his absence due to rheumatiod arthritis but on 9/30/05 we received a MRI report showing a small rotator cuff tear in his shoulder.

EE WAS INSTALLING A DIFFERENTIAL ON A CAT 793 OFF-HIGHWAY TRUCK. RATHER THAT USING A LADDER TO COME DOWN FROM THE TRUCK'S A-FRAME, EE JUMPED TO THE GROUND, 3-4' BELOW. WHEN EE LANDED HE FELT A SHARP PAIN IN HIS ABDOMEN. BECAME REPORTABLE ON 1/21/05 WHEN EE WENT TO DOCTOR & WAS DIAG. AS HAVING HERNIA - SURGERY SCHEDULED. PHYSICIAN'S RECOMMENDATION TO NOT RTW PRIOR TO SURG.

Employee was stepping off a lowboy trailer and lost his footing. As he fell he struck his left elbow on the trailer hitch. This resulted in a broken head of the radius requiring surgery to repair.

Employee stepped off the bumper of his service truck onto uneven ground and twisted his ankle. He continued to work until December 1st when he was seen by a doctor. He was diagnosed with an avulsion fracture in his left ankle and must wear a walking boot for two weeks.

Employee was removing a long bolt from a propel cam on the dragline. The bolt came out and the employee caught it with his left hand, pulling his shoulder. As he started down the stairs, he fell and ended up on the dragline shoe. He received a broken humerous, dislocated shoulder and a laceration to his right leg requiring sutures.

Injured employee was on third step of six foot step ladder cutting the top pin holding a strut. Bottom pin was removed. When top pin was cut, bottom of strut swung out and hit ladder knocking employee off onto the ground. Strut was secured by a choker attached to a boom on service truck.

He stated that after dismounting from battery ram car, he slipped on a loose piece of draw rock ont he mine floor, causing him to fall to the ground. as he was falling, he tried to catch his balance with his right hand, causing soreness & swelling. he continued to work and had a specialist diagnose it on 10-27-04 as having a fracture to the 4th metacarpal bone in his right hand.

Employee fell from the bumper of his service truck. He was attempting to raise the hood and was using the grill section as a grab handle. The grill section broke away, causing him to loose his balance and fall onto the concrete shop floor. He has contusions to his lower back and left arm.

EE WAS TIGHTENING LUG NUT ON SCOOP TIRE WHEN THE SOCKET SLIPPED OFF THE LUG NUT. EE WAS PUTTING DOWN PRESSURE ON THE 3/4" RATCHET WHEN THE SOCKET SLIPPED. HE LOST HIS BALANCE AND FELL FACE-FIRST INTO THE LEFT REAR FENDER OF THE SCOOP.

Employee was walking back to his drill when he slipped on a patch of ice and fell down on his left elbow, jamming his arm up into his shoulder. He initially thought he would be OK. He was seen by an Orthopedic Surgeon in May and diagnosed with a torn rotator cuff. He had surgery on May 25 to repair the tear and began losing time from work on that day.

EMPLOYEE WAS CUTTING A SECTION OF BEAM OUT OF THE FLOOR STEEL FOR REPLACEMENT. WHEN THE CUT WAS COMPLETED, THE BEAM SHIFTED. HE JUMPED BACK, LOSING HIS BALANCE, FALLING APPROXIMATELY 4' TO THE FLOOR BELOW. HE WAS WEARING A HARNESS THAT WAS CONNECTED TO THE STEEL BUT AT FLOOR LEVEL. THE LANYARD WAS NOT CONNECTED HIGH ENOUGH TO PREVENT THE FALL.

EE WAS WALKING ON THE WALKWAY BETWEEN THE CHANGE HOUSE AND THE TRUCK SHOP PRIOR TO THE START OF THE SHIFT. THE WALKWAY WAS SLICK. HE SLIPPED AND FELL INJURING HIS LEFT KNEE. HE WAS DIAGNOSED W ITH A POSSIBLE TORN QUADRICEPS MUSCLE IN THE LEFT KNEE. HE WAS PLACED IN A LEG BRACE, LIMITING THE RANGE OF MOTION AND RETURNED TO HIS NORMAL DUTIES THE SAME DAY.

EE WAS PULLING THE FUEL HOSE TO A TANK ON AN AUGER. WHEN HE PASSED THROUGH A NARROW SPACE BETWEEN THE SCALE HOUSE AND AUGER HE TURNED & PULLED THE HOSE BACKWARD. IN DOING SO HE TRIPPED & FELL.
