While opening a drawer in a tool box mounted on the catwalk, employee stepped backward and put foot under guard rail. He fell against the handrail striking his back and ribs, fracturing a rib.
Employee was raising the hood on truck #107. He lost his footing and fell backward off of the fender and landed on his right hand breaking his hand or finger.
Employee was getting down off his haul truck to use the restroom. He was on the bottom step and slipped and caught himself with his right hand and felt a pop in his right shoulder. The diagnosis was a torn bicep.
Employee went up the engine ladder to turn heater valves on. He slipped on the ladder and hit his left elbow.
At the time of the incident EE was climbing down the boarding ladder of a water truck. When He reached the bottom of the ladder he extended his right toward the ground and hopped off the ladder.Upon landing his right knee twisted and he fell to the ground.
Employee lost his balance and jumped from D9 dozer walkway resulting in a fracture to his right heel.
He was crossing over a mobile bridge system and tripped and fell. He continued to work and started missing work on 8/30/05. The original doctor's report showed his absence due to rheumatiod arthritis but on 9/30/05 we received a MRI report showing a small rotator cuff tear in his shoulder.
HE WAS WALKING UP AN ENTRY AND STEPPED IN A HOLE AND TURNED HIS ANKLE.
Coming down ladder - foot slipped on step - step wet from water - went backward & caught herself with right hand - sprained right wrist.
Slipped on step as walking up.
EE informed supervisor. toehr on that he tripped over a shank, causing him to fall on another shank, injuring rt. side and lower back.
Slip & trip fall from bed of trailer, ee landed on his back on the ground. rib bruise/contusion to right mid back.
Employee tripped over tire and fell (torn ligament in shoulder - surgically repaired on 6/2/05).
Employee stepped in an open 8" drill hole, fell backwards to the ground and his foot became stuck. His foot was quickly dislogded from the hole and he suffered strains/sprains to his right hip and lower back.
Employee fell at entry to door to changehouse as he was reporting for work. The door is fifteen feet from the main gate. He was not on the payroll at the time. He broke his ankle. He says there was ice.
EE WAS INSTALLING A DIFFERENTIAL ON A CAT 793 OFF-HIGHWAY TRUCK. RATHER THAT USING A LADDER TO COME DOWN FROM THE TRUCK'S A-FRAME, EE JUMPED TO THE GROUND, 3-4' BELOW. WHEN EE LANDED HE FELT A SHARP PAIN IN HIS ABDOMEN. BECAME REPORTABLE ON 1/21/05 WHEN EE WENT TO DOCTOR & WAS DIAG. AS HAVING HERNIA - SURGERY SCHEDULED. PHYSICIAN'S RECOMMENDATION TO NOT RTW PRIOR TO SURG.
Employee was stepping off a lowboy trailer and lost his footing. As he fell he struck his left elbow on the trailer hitch. This resulted in a broken head of the radius requiring surgery to repair.
Employee stepped off the bumper of his service truck onto uneven ground and twisted his ankle. He continued to work until December 1st when he was seen by a doctor. He was diagnosed with an avulsion fracture in his left ankle and must wear a walking boot for two weeks.
Employee was removing a long bolt from a propel cam on the dragline. The bolt came out and the employee caught it with his left hand, pulling his shoulder. As he started down the stairs, he fell and ended up on the dragline shoe. He received a broken humerous, dislocated shoulder and a laceration to his right leg requiring sutures.
Mechanic stepped on a piece of coal or dirt, lost his balance and struck his forehead on an open door of his service truck. He received a laceration that required sutures to close.
Injured employee was on third step of six foot step ladder cutting the top pin holding a strut. Bottom pin was removed. When top pin was cut, bottom of strut swung out and hit ladder knocking employee off onto the ground. Strut was secured by a choker attached to a boom on service truck.
He stated that after dismounting from battery ram car, he slipped on a loose piece of draw rock ont he mine floor, causing him to fall to the ground. as he was falling, he tried to catch his balance with his right hand, causing soreness & swelling. he continued to work and had a specialist diagnose it on 10-27-04 as having a fracture to the 4th metacarpal bone in his right hand.
Employee was climbing up ladder to operator's cab of auger and slipped. Left index finger got caught while attempting to grasp a rail.
Employee walked over an embankment as short cut to the equipment parking area. He fell striking his left forearm resulting in a fracture.
EMPLOYEE SLIPPED AND DID THE SPLITS WHEN GETTING OFF MACHINE WHICH SHE HAS DONE WITHOUT INCIDENT MANY TIMES BEFORE.
After boarding the 16G grader, the employee lost his balance, turned and jumped to the ground. He sprained his ankles and left shoulder. * Note - Employee did not start missing work until 10/6/04.
HE TWISTED HIS KNEE AS HE STEPPED DOWN FROM A SCOOP. HE CONTINUED TO WORK & BEGAN MISSING ON 10/22/04 TO REPAIR TORN CARTILAGE IN HIS KNEE.
Employee was climbimg a 6-foot step ladder and carrying an impact wrench when the ladder kicked out from under him. The employee landed on his feet, then fell backwards and his hand struck his safety glasses, causing a laceration above his left eye requiring sutures to close.
Employee fell from the bumper of his service truck. He was attempting to raise the hood and was using the grill section as a grab handle. The grill section broke away, causing him to loose his balance and fall onto the concrete shop floor. He has contusions to his lower back and left arm.
BASED UPON THE INFORMATION RECEIVED AT THIS TIME, EE WAS PUTTING A SPRING ON EE'S TRUCK AND HE FELL OFF AND HIT HIS RIGHT ARM AND IT STARTED TO SWELL. THE DR SAID THAT HE BRUISED A MUSCLE AND WOULD BE BACK TO WORK ON THE FOLLOWING MONDAY. EE WENT BACK TO THE DR. ON MONDAY AND CALLED AND SAID THAT HE WOULD BE OFF LONGER THAN ORIGINALLY ANTICIPATED
Employee was ejected from a hydraulic manlift and fell approximately 12-14 feet to the ground. He received fractures to his left shoulder, left knee and left femur.
EE WAS TIGHTENING LUG NUT ON SCOOP TIRE WHEN THE SOCKET SLIPPED OFF THE LUG NUT. EE WAS PUTTING DOWN PRESSURE ON THE 3/4" RATCHET WHEN THE SOCKET SLIPPED. HE LOST HIS BALANCE AND FELL FACE-FIRST INTO THE LEFT REAR FENDER OF THE SCOOP.
Employee was cutting trees. When walking from one tree to another he fell and hurt knee. Crew leader notified me of injury on 6-14-04. EE finished shift, went to Dr after work and injury was ruled a swollen knee.
EE TRIPPED ON A SMALL MOUND OF DIRT. AS HE TRIED TO REGAIN HIS BALANCE HIS RIGHT FOOT TWISTED, CAUSING HIM TO FALL IN A SITTING POSITION WITH THE MAJORITY OF HIS BODY WEIGHT LANDING ON HIS RIGHT ANKLE.
EE states reaching into cab fdr, cb other backstead to grab handle and missed fell and landed on my back. the ee was examined x ray any provide to have and discerable injury. ee refeaered for follow up and failed to provide us with any documention sending it as a lost time injury. we continued to question the validity of the incident.
INJURED JUMPED OFF A TRAILER - AS A RESULT HE SPRAINED HIS BACK.
Employee was walking back to his drill when he slipped on a patch of ice and fell down on his left elbow, jamming his arm up into his shoulder. He initially thought he would be OK. He was seen by an Orthopedic Surgeon in May and diagnosed with a torn rotator cuff. He had surgery on May 25 to repair the tear and began losing time from work on that day.
EMPLOYEE HAD BEEN OPERATING PORTAL BUS AT STARTING TIME. HE GOT OFF BUS TO RELEAVE HIMSELF & UPON RE-ENTRY TO BUS HE SLIPPED & FELL ON ICY SURFACE, LANDING ON HIS LEFT SHOULDER.
EMPLOYEE WAS CUTTING A SECTION OF BEAM OUT OF THE FLOOR STEEL FOR REPLACEMENT. WHEN THE CUT WAS COMPLETED, THE BEAM SHIFTED. HE JUMPED BACK, LOSING HIS BALANCE, FALLING APPROXIMATELY 4' TO THE FLOOR BELOW. HE WAS WEARING A HARNESS THAT WAS CONNECTED TO THE STEEL BUT AT FLOOR LEVEL. THE LANYARD WAS NOT CONNECTED HIGH ENOUGH TO PREVENT THE FALL.
WHILE ATTEMPTING TO MAKE A DELIVERY, THE EE WAS REQUIRED TO WADE THROUGH 12-14" OF STANDING WATER. IN DOING SO HE TRIPPED OVER A SUBMERGED CABLE. WHEN HE FELL HE STRUCK HIS LT. KNEE ON A ROCK FRACTURING THE PATELLA. HE ALSO SPRAINED HIS LT.SHOULDER & BOTH ELBOWS IN ARRESTING THE FALL. HE SOUGHT TREATMENT AT 5:00PM.
EE WAS CONDUCTING A PRE-SHIFT EXAM OF THE WATER TRUCK. AS HE WAS ON THE GROUND & WALKING AROUND THE TRUCK, HE SLIPPED & CAUGHT HIMSELF. HE IMMEDIATELY FELT PAIN DOWN HIS RT. LEG. EE REFUSED TO SEEK MEDICAL ATTN. & COMPLETED SHIFT. THE EE WORKED HIS NEXT 2 SCHEDULED SHIFTS. AFTER HE WAS OFF ON SUNDAY, HE CALLED IN ON MONDAY, STATING THAT HIS BACK HAD WORSENED & HE NEEDED MED.ATTN.
EE WAS WALKING ON THE WALKWAY BETWEEN THE CHANGE HOUSE AND THE TRUCK SHOP PRIOR TO THE START OF THE SHIFT. THE WALKWAY WAS SLICK. HE SLIPPED AND FELL INJURING HIS LEFT KNEE. HE WAS DIAGNOSED W ITH A POSSIBLE TORN QUADRICEPS MUSCLE IN THE LEFT KNEE. HE WAS PLACED IN A LEG BRACE, LIMITING THE RANGE OF MOTION AND RETURNED TO HIS NORMAL DUTIES THE SAME DAY.
EE WAS WALKING AWAY FROM WORK AREA & EXPERIENCED PAIN IN HIS RT. CALF. NOTE: EE DID NOT START MISSING WORK UNTIL 11/2/03.
EMPLOYEE FELL APPROXIMATELY 15 FEET FROM THE SHOVEL CATWALK WHILE CLEANING HANDRAILS.
HE STATED THAT AS HE WAS OBSERVING THE MINE ROOF, HE ATTEMPTED TO STEP UP ONTO THE WALKWAY OF THE ROOF BOLTING MACHINE. HE TRIPPE ON THE REAR HITCH AND FELL ONTO THE WALKWAY, HITTING HIS RIGHT KNEE ON THE WALKWAY. IT WAS DIAGNOSED AS AN "AVULSION OFF THE SUPERIOR POLE OF THE RIGHT PATELLA" (FRACTURE).
EE GOT OFF A GRADER AND WAS WALKING INTO THE FUEL STATION WHEN SHE SLIPPED ON AN OILY AREA. SHE CAUGHT HERSELF AND FELT A SHARP PAIN IN HER LOWER BACK. SHE WAS SEEN BY A DOCTOR ON OCT 10TH, WH O DIAGNOSED HER WITH A SPRAINED BACK. SHE CONTINUED TO EXPERIENCE PAIN & WAS SEEN BY AN ORTHOPEDIST WHO ORDERED AN MRI WHICH REVEALED SHE HAS DEGENERATIVE DISC DISEASE AGGRAVATED BY HER SLIP
EE WAS PULLING THE FUEL HOSE TO A TANK ON AN AUGER. WHEN HE PASSED THROUGH A NARROW SPACE BETWEEN THE SCALE HOUSE AND AUGER HE TURNED & PULLED THE HOSE BACKWARD. IN DOING SO HE TRIPPED & FELL. THIS WAS REPORTED TO HIS SUPERVISOR THE NEXT DAY AS AN INCIDENT WITH NO LOST TIME OR MEDICAL TREATMENT. EE DID NOT REPORT UNTIL WORKERS' COMP CLAIM ARRIVED.
EMPLOYEE CLIMBING DOWN THE STAIRS OF AUGER TRUCK, SLIPPED ON STEP, HIT RIGHT SHIN ON STEP. RESULT - CONTUSION OF RIGHT SHIN (AREA BELOW RIGHT KNEE).
EE WAS EXITING A BULK TRUCK WHEN HIS RIGHT ANKLE TWISTED AS HE CONTACTED A STEP ON THE VEHICLE CAUSING AN ANKLE SPRAIN. THE EE WAS IN COMPLIANCE WITH MINE RULES AND REGULATIONS. THE ACCIDENT DID NOT INVOLVE THE MINING SYSTEM. THE ACCIDENT DID NOT INVOLVE JOB SKILLS OR PROTECTIVE ITEMS.
EMPLOYEE FELL WHILE DISMOUNTING PORTAL BUS, STRAINING HIS NECK AND LEGS.
