Employee was accessing a track excavator when ee slipped and lost footing. Employee was seen at medical the next day and released for next scheduled shift.(3/21/2019) with no restrictions. Employee went back to medical on 3/21/19 and was released with restrictions making this a reportable injury. We are reporting within the 10 day requirement from 3/21/19

The employee was exiting a pickup and tripped and fell out onto the ground. The foot was tangled in wiring fed through the passenger door. After medical review it was determined that the right ankle was fractured and that EE would need to wear a boot for three (3) weeks.

Employee was attempting to repair a support on a float barge when EE's foot slipped and EE fell over onto hip causing a bruised hipbone.

The employee called control room to shut off 2 belt through 4 belt to inspect the sampler return chute. The employee climbed up to stand on the hand rail and leaned over the walkway using hands to steady themselves on the Chute wall. The employee Inspected the chute, went to push off and slipped causing their hand to slip into the chute causing the injury.

An employee was walking a belt line and EE slipped mid step, EE felt pain in EE's back.

Employee was exiting training room for break. Employee's foot went under the edge of the floor mat that had curled up causing the employee to fall. Left ankle was dislocated and fractured.

While repairing the windshield washer system on a crane, the employee attempted to enter the cab from the main frame platform, lost grip and footing and dropped 6 feet to the ground. The employee landed on feet but sustained a twisted ankle and broken heel bone upon landing.

While climbing off the 703 water truck the operator missed the hand rail and the bottom step, falling backwards on to the ground. The operator felt pain in their left lower back and hip area.

An employee was jerking on ventilation curtain that was tangled in miner cable. The employee stated ee went to take a step after retrieving the curtain and felt pain in back that caused ee to drop to ee knees.

Employee was standing on front platform helping a mechanic bolt a light bracket on the lift cylinder of a D11 bulldozer after they had replaced Hyd. hoses. The employee lost balance and handhold causing EE to fall. Employee received a cut on the head that required stitches.

Employee was getting a fuel hose to fuel a loader and slipped on ice and snow pack, employee stuck hand out to catch self. Employee's hand landed up against the ladder bending index finger back and fracturing it.

Employee was climbing down off Highwall Miner and either slipped or missed a step and fell to the deck and then onto the ground. Employee broke employee's leg right above employee's ankle in the fall.

Employee had taken a shower at the end of EE's shift. EE was walking from the shower to EE's locker when EE slipped. EE's right hand caught a sharp edge of a locker when EE fell. EE's right hand palm received an open wound requiring six (6) stitches to close. EE reported to work at the mine on EE's next scheduled shift. No time Lost!!!

Operator noticed that a culvert was plugged up on one end so ee got off machine to go and shovel it out. On ee way ee twisted ankle on uneven ground on the pit floor. Operator worked the rest of the shift but the next morning ankle was swollen so ee called off work and went to the doctor.

Employee reported to line-out meeting at beginning of the shift and was attending a safety presentation that was to be 1 1/2 hours in duration. 30 minutes into the training, the employee was not feeling well and was going to go get some fresh air. The employee stood up and fainted and dropped to the floor. This resulted in a non-displaced hairline fracture of upper jaw.

Employee was evaluating a stuck vehicle and was walking to the rear of the vehicle and tripped on a windrow of rocks. While stumbling forward, the employee planted their right foot and felt a pop in their right knee.

EE was getting off a motor grader by going down the ladder left foot remained on the ladder and ee stepped onto the moldboard when right foot slipped and ended up doing splits resulting in a pulled groin muscle in left leg.

Operator was boarding a dozer and slipped and fell to the ground.

Mechanic was dismounting a D10 Dozer and as employee turned to face the machine and climb down employee lost employee's footing and fell.

Employee was spotting a drill being loaded on a transport when they took a miss-step and slipped off the side of the transport. The employee cut their right shin and required stitches.

Employee was tying in presplit holes on the blast pattern and crossed over a presplit line, miss-judged the hole location and stepped into the hole with left leg. EE was diagnosed with a non-displaced fracture in left knee that required surgery to repair.

Employee was walking on loose dirt, stumbled, and jammed right foot into a small berm. X-ray revealed that an existing bone spur was broke off. EE could not get their boot on for the next shift so this became a Lost Time Incident.

While dismounting from a motor grader the employee shifted their weight while one foot was still on the ladder. This caused EE's hip to twist and EE felt something pull in their right inner thigh.

Tripped while walking in the return. Tried to catch self by grabbing the rib mesh. Right middle finger was caught in the mesh and resulted in a fracture and laceration.

Lost balance and fell forward while walking along entry. Reached out with right arm and caught himself, causing pain in right shoulder

Employee was walking to dozer and slipped on a patch of ice. This was not reported until 1/25/16 and Workers' Comp conducted an investigation. The claim was approved on 2/11/2016. Employee missed three days with a lower back sprain.

EE was walking out of the maintenance shop in the dark & fell on a patch of ice covered from overnight snow. EE landed on EE's left side & arm. EE declined medical attention at the time. EE had a doctor's appointment for a physical on 12/29/2015. During the physical an x-ray found one of EE's ribs was fractured, thus the reason this report wasn't filed at the time of the incident.

EE was using a socket to tighten a lid on a Hyd. filter lid and the wrench slipped causing the EE to lose his balance and fall.

The employee was leaving Mine Medical on her way to the warehouse. The employee was stepping off of the last step of four steps when her knee gave out. She tried catching herself unsuccessfully and fell to the ground contacting her face onto the ground. This caused a chipped tooth, facial abrasions and contusion on her left knee.

Stepping down off of the bed of a truck ee stepped onto the step installed above the receiver hitch. His right foot slipped off of the step & fell off, hitting his back on the step. He did not seek medical care until 10/31/15 at which time it was discovered that he had 2 fractured ribs.

EE found lying on the ground by a D11 dozer. EE alleges he was about to climb onto the dozer slipped & fell. EE does not know what happen. EE is a large person & was complaining of severe headache, dizziness, nauseous & discomfort in the lower back. Company believes that a medical or medication condition caused EE to lose EE's balance and fall. Waiting for medical results.

Employee was dismounting a motor grader when his right foot got caught in the ladder causing the employee to hyper extend his right leg. This caused a pulled groin muscle in his right leg.

While rappelling during a Mine Rescue Training, the employees left knee twisted when they contacted the ground. The employee did not seek any medical attention for several weeks. The ACL was torn and required surgery on 9/16/15. Work activities were not restricted until that date.

EE statement: I was tightening a joint of conduit with a 12" pipe wrench and my footing slipped causing my body to twist hurting my shoulder. Pain in my shoulder, Rt shoulder.

EE stepped off of a raise platform(8" high)to the floor of the 1570's house. When EE stepped off the raised area EE heard a pop in the L knee. Knee x-rayed, inconclusive. EE returned to work on restricted duty awaiting a MRI. EE due to swelling was unable to work on 03/20/2015 & will return after the MRI pending the results. To date no medical treatment has been performed.

EE was doing a pre-shift on a blade and while dismounting from the OEM boarding ladder the EEs foot slipped when he stepped to the ground and he hyper-extended his right knee

Cleaning Service lady was taking garbage to a dumpster when she slipped & fell backwards hitting her head on the ground. She had walked to the wrong dumpster - a little used scrap metal dumpster not used for regular garbage. Light snow covered a layer of ice.

An employee stepped in a parking ditch causing his knee to pop and causing pain. The employee was seen by an orthopedic specialist and an MRI was done on 10/17/14. This was the soonest he could be seen. He was released back to work on restricted duty on 10/17/14.

Stepped on unsecured scaffolding plank which shifted causing him to fall approximately five feet onto an I-Beam. Resulted in five fractured ribs and hemothorax.

EE was pulling fuel hose when it got caught on something and spun him around wrenching his lower back. A physician diagnosed a lower back sprain and the employee began missing work on 10/10/2014 he received a full release from the doctor on 10/14/2014. Workers Comp questioned the claim and approved this as work related on 11/27/14.

Employee was pulling a suction hose and float from a pond when the employee slipped on a muddy area, fell and twisted his lower back

An employee was descending a platform ladder and slipped on the third step down causing him to twist his ankle. The employee resumed work without informing his supervisor of the incident. The employee had the ankle checked the next day and X-rays revealed that it was broken.

Employee was operating the watering truck and said he might have slipped in the mud after he had exited the truck and was walking around. He went to the ER on the evening of 6/1/14. ER prescribed pain pills and told him he could not return to work for at least 4 days.

The employee stepped onto the deck of an overburden drill that was wet due to rain and slipped. This caused him to twist his knee. The knee required surgery 20 days later.

End of shift EE was dismounting Kress coal hauler. His right foot was on the ground and his left foot became stuck on the step. EE twisted to free left leg, which is a prosthetic device, and twisted his right knee. Required ice and elevation and missed two shifts.

Employee stepped backwards off bottom rung of 3' stepladder injuring right knee. Event was not reported for 5 days and there were no witnesses. Medical evaluation was strain to knee. Report was given to Montana State fund and on 12/06/2013 Montana state fund determined it was a work related injury

Maintenance EE mounting a scraper at the rear engine to check engine. EE missed a step causing EE to lose balance & fall. EE landed on right wrist which was fractured.

The employee was walking on the deck of a haul truck with both of her hands full when she tripped and fell. When the employee fell, her head came into contact with the coolant reservoir tank causing a slight fracture to the employee's forehead.

On May 21, 2012 an employee was assigned duties as a laborer in C Shop. The employee was in the process of getting on a floor sweeper. His left leg was on the sweeper and his right leg was on the floor. When he started to get on the sweeper he heard a loud pop emanate from his right knee.

Employee got his arm caught in the door of a pick up and fell while trying to stop it from rolling. This caused his right radius and ulna to be fractured.
