Employee sustained a left shoulder dislocation due from a same level fall in slick muddy conditions from ground warming as the day progressed. Employee did not recognize the change in conditions and slipped and fell onto their shoulder.

EMPLOYEE WAS EXITING BULK TRUCK AND TWISTED AND SPRAINED AN ANKLE.
While retrieving a cardboard box employee stepped out of the back seat of a pickup and rolled left ankle on a ~ 3" piece of scoria.
Employee's foot slipped in the truck cab door way while exiting a 6x6 water truck. EE landed on the ground with weight on ee left foot, resulting in a fracture to left heel.
Employee was washing boom off, lost track of where EE was standing and slipped and fell striking the right side of EE's head. Employee appeared to bleed from the nose and the onsite safety supervisor took precautionary measures and called ambulance.
Employee was walking on a downward grade on loose ballast, (small rocks along railroad tracks) the ballast slid causing the employee to fall backwards, as employee was falling employee put right arm out to brake fall, impacting hand and breaking right wrist.
Engineer stepped out of pick up on reclamation project, he stepped into a low spot and fell a twist in he knee. He did not recline the step was going to be as low as it was.
Employee tripped and fell when a plywood walkway at ground level shifted. She landed on her right shoulder and hand. Hospital confirmed no fractures and prescribed pain medication. Shoulder specialist diagnosed it as bone contusion on shoulder and prescribed physical therapy and pain medication. Unable to use arm 6 to 12 weeks to heal.
Grounds person was attempting to move a table, while doing so he tripped on a 5 gallon water bottle dropping the table and falling at the same time, he then hit his eye brow on edge of table causing a cut that was glued together at the Campbell County walk in clinic.
Utility person was setting up an air compressor to pig a discharge line on a sump. He stumbled on a piece of scoria rock and fell down against the sump sign marking the sump. His left knee contacted the corner of the base of the sign and cut his knee. The safety supervisor and a rescue team member transported him to the emergency room.
An employee slipped in the change-house causing a laceration to his hand requiring 3 stitches
The employee slipped on a step while he was descending the ladder of a haul truck. When he slipped, he felt a sharp pain in his lower back. The Dr. found nothing and told the employee to take otc Tylenol. The employee missed 1 shift.
An operator stepped out of the pickup and rolled his ankle. He felt pain immediately and his ankle started to swell. He was taken to the hospital for x-rays which were negative. - This is an update on an earlier Medical First Aid case. The injured person saw a specialist today (08/10/2012) who determined that there was a break in an ankle bone that was not detected earlier.
Mechanic was hauling hydraulic filter down a ladder and slipped. Falling backwards off the ladder his watch caught on a bolt causing a laceration of his left wrist,
Electrician doing pm on shovel. He went to step off a step and missed, he hit the left side of his head on a steel beam causing a laceration along his eye brow. Individual needed stitches to close the laceration. He was released back to work with no restrictions.
On August 12th, the injured employee was getting out of a boat. The employees foot caught the lip of the boat and he fell. He hit the concrete spillway and jammed his shoulder. His shoulder continued to hurt and was taken to the Med Clinic for x-rays on August 22nd and was given a prescription for the pain.
The injured person was assisting a maintenance employee to change a tire. The injured person reached to pick up lug nuts. His foot slipped. He was seen by the mine's Emergency Response Team and then taken to the Emergency room for x-rays where it was determined that there was a broken bone in his ankle.
Warehouse tech was pulling wrapping paper out of freight box. When paper came undone, he stepped backwards and tripped on pallet causing him to fall. Contusion on right hip.
Extension cord was run from laboratory through hallway and plugged in by water cooler. A large floormat was placed over the extension cord to use for a safe walking path. The technician walked around the floor mat where the extension cord was exposed, stepped on extension cord, rolling her foot. Technician fell backwards hurting her back and bumping her head.
Employee was walking across some hard troweled snow covered concrete when he lost his footing and slipped and fell strking his left shoulder.
The injured employee slipped off the deck of a D-9 Dozer onto the tracks, landing on his tailbone which caused a muscle spasm in his lower back
EE WAS DISMOUNTING A 5230 SHOVEL WHEN THE LADDER GOT HUNG UP AND CAUGHT HIS FOOT BETWEEN RUNGS CAUSING STRAINED LIGAMENTS IN HIS LEFT FOOT.
