He went to the doctor on Sunday Feb 12. He did not notify his supervisor until Feb 23 that he had had medical attention.

He thought it had only sprained his left wrist, and did not go to the hospital. However the wrist did not improve so he went to the doctor on January 6, 2005. His left wrist was broken.

HE WAS FILLING TANK WITH WATER. WENT TO GET DOWN OFF OF THE TRUCK AND THE PIECE OF METAL HE WAS HOLDING ONTO GAVE LOOSE, CAUSING HIM TO SLIP, CAUSING HIM TO HIT THE BACK OF THE TRUCK WITH HIS LEFT HIP AND CUTTING THE PALM OF HIS LEFT HAND.
AFTER SERVICING D11N AS HE WAS GETTING OFF OF DOZER HE SLIPPED AND LEG WENT BETWEEN TRACK AND PUSH ARM.
EMPLOYEE SAID WHEN HE WAS GETTING OFF HIS TRUCK HE MISSED THE BOTTOM STEP HE LANDED ON HIS RIGHT HIP. EMPLOYEE WENT TO HOSPITAL IN AN AMBULANCE X RAYS INDICATED A BROKEN HIP AND PELVIS.
FALL WHILE DISMOUNTING HAULER. ALL HANDRAILS & STEPS WERE IN GOOD SHAPE.
EE WAS EXITING HIS LOADER AT THE END OF THE SHIFT WHEN HIS KNEE GAVE AWAY
THE EE HAD RETURNED TO CHECK SHOT AFTER BLAST. WHEN WALKING ACROSS SHOT HE STEPPED ON A ROCK THAT GAVE AWAY CAUSING HIM TO FALL.
HE WAS GETTING ON THE EXCAVATOR WHEN HIS RIGHT KNEE GAVE OUT ON HIM AND CAUSED HIM TO FALL.
OPERATOR WAS EXITING MACHINE, DESCENDING FRONT LADDER WHEN HE SLIPPED. THIS RESULTED IN A FALL OF APPROX 8' TO THE GROUND, WHERE HE LANDED ON HIS BACK, ARM AND HIP. FOREMAN TRANSPORTED OPERATO R TO NORTON COMMUNITY HOSPITAL WHERE DR EXAMINED AND RELEASED HIM. AN APPOINTMENT WITH OCCUMED WAS SCHEDULED 10/3/00.
EE WAS WALKING TO HIS BLASTING TRUCK AFTER CHECKING A SHOT FOR BLASTING. HE STEPPED ON A ROCK THAT TURNED UNDER HIS FOOT CAUSING HIM TO FALL. THE EE REPORTED TO ME HE HAD INJURED HIS RIGHT FOO T. HE WAS THEN TAKEN FOR MEDICAL ATTENTION.
STEPPED ON A ROCK AND FELL HITTING A PIECE OF METAL. RIGHT FOOR WENT UNDER THE EDGE OF SOME METAL.
