Employee was walking across a belt crossing on the surface of the mine when he slipped and fell striking his right arm. He was transported to the hospital where it was determined that he had no fractures and was to return to work the next regular shift. Employee did not report for duty and now is claiming to have a back injury from the accident.
Employee was walking to his service truck and apparently blacked out. He stated that he did not remember falling and did not trip. He was transported to the hospital where he received 3 stitches to his lip. He returned to work 5/2/05 his next scheduled shift.
EE stated he was hanging curtain in the mine when he slipped and twisted his knee. ee continued to work until 8-6-04 when he condsulted a dr.
EMPLOYEE STATED HE WAS CHANGING BATTERY LEADS ON THE SCOOP AND WAS ON HIS HANDS AND KNEES ON THE SCOOP. HE STATED HIS HANDS SLIPPED AND HE FELL OFF THE SCOOP STRIKING HIS FOREHEAD ON THE BAT TERY STAND. HE WAS TRANSPORTED TO THE HOSPITAL WHERE HE RECEIVED 19 STITCHES.
EMPLOYEE STATED HE WAS GETTING INTO THE SCOOP BUMPED HIS KNEE. EMPLOYEE CONTINUED TO WORK UNTIL MEETING WITH DOCTOR ON 7-22-02.
EE WAS IN THE SCOOP BUCKET & FELL DOWN STRIKING HIS RIGHT KNEE, DISLOCATING IT.
EMPLOYEE CLAIMS THAT AS HE WAS INSTALLING A RIB BOLT HE SLIPPED AND IS LEFT KNEE STRUCK A PIECE OF METAL ATTACHED TO THE BOLT MACHINE CAUSING PAIN AND SWELLING TO THE AFFECTED AREA.
THE VENTILATION DOORS OUTSIDE, EE, A CONTRACTED SECURITY GUARD, SLIPPED ON THE TRACKS AND FELL, STRIKING HER RIGHT WRIST AGAINST THE OPPOSITE TRACK.
EMPLYEE STATED THAT HE WAS STEPPING ACROSS THE TONGUE OF THE RAIL RUNNER WHEN HIS FOOT SLIPPED RESULTING IN A BROKEN BONE JUST ABOVE THE ANKLE.
EE STATED THAT HE WAS CLEANING THE HEAD DRIVE WHEN HE SLIPPED AND HIS LEFT ANKLE AND HIT HIS LEG ON THE TAIL PIECE HE CONTINUED TO WORK UNTIL 10/30/00 WHEN HE CONSULTED A DOCTOR AND LOST TIME BEGAN.
HE STATED HE WAS WALKING WHEN HE FELL AND STRUCK HIS RIGHT KNEE ON A TRACK TIE. HE CONTINUED TO WORK UNTIL 7/25/00, WHEN STARTED TO LOSE TIME AND CONTINUES TO BE A LOST TIME AS OF 8/7/00. HE WAS DIAGNOSED WITH A BRUISED KNEE.
EE STATED THAT HE WAS APPROX. 2 CORSSCUTS UNDERGROUND WHEN SLIPPED ON ICE AND FELL HITTING HIS HEAD, BACK AND HIP.
EE STATED HE WENT TO THE BACK OF THE ROOF BOLTER TO GET SOME GLUE WHEN HE SLIPPED AND TWISTED HIS LEFT KNEE.
