EE was thrown from equipment, resulting in a dislocation to his right shoulder. EE has since then been referred to an Orthopedic Surgeon for possible surgery and will be off work on Workmans Comp for an uncertain period of time.

EE was carrying belt rollers through the crosscut when he tripped on metal band canding falling onto left shoulder. EE continued to work until 09/10/09 when he had surgery on shoulder.

Stepping down from drop deck of semi-trailer, lost footing, fell and broke right knee and leg.

EE WAS DOING REPAIRS IN #2 SUMP. HE LOST HIS FOOTING WHILE MOVING AND FELL. AREA WAS WET AT THE TIME.

Employee was replacing a cover over main breaker panel on the battery scoop. As he was replacing the cover he lost his footing causing him to drop the lid on his hand.

EE WAS WALKING WHEN HE STARTED TO FALL AND TWISTED RIGHT KNEE. EE CONTINUED TO WORK AFTER TWISTING KNEE. MRI TEST REVEALED BONE SPUR ON RIGHT KNEE REQUIRING ARTHROSCOPIC SURGERY ON 3/7/05. ACCIDENT BECAME A LOST TIME INJURY ON 3/7/05.

CLIMBED ON TO TRAILER TO CHECK TARP, THEN JUMPED OFF & SUSTAINED A SIMPLE FRACTURE OF LEFT LEG.

EE WAS WALKING DOWN A SET OF THREE STEPS WHEN HIS HEEL SLIPPED ONT HE 2ND STEPS, THEN HIS HEEL STOPPED SLIDING CAUSING HIM TO FALL FORWARD. HIS FOOT FOLDED UP UNDERNEATH HIM CAUSING A SPRAIN T O HIS LEFT FOOT. THE STEPS WERE WET AND SLICK FROM THE RAIN.

EE WAS WALKING ALONG BELT LINE CARRYING A 4'LONG TIMBER WHEN HE SLIPPED & THEN FELT A BURNING SENSATION IN HIS LEFT HIP.

CRANE OPERATOR SLIPPED AND FELL WHILE DESCENDING FROM OPERATOR'S CAB.

EMPLOYEE JUMPED TO THE MINE FLOOR FROM AN ELEVATED POSITION, UPON LANDING HIS ANKLE ROLLED UNDER. EMPLOYEE SUSTAINED A FRACTURE TO THE RIGHT ANKLE.
