Employee was climbing off excavator and slipped on the track of the excavator hitting their right shoulder and right elbow. Employee worked from the date of incident till they had shoulder surgery on 05/29/2023.

Employee was climbing ladder that leads to mix tank platform. Foot slipped and employee tried to keep from falling by grabbing fencing - Hand was cut on fencing.

The employee exited the dozer to look over the berm where EE was working. While attempting to walk up the berm, the employee reports EE lost footing and stumbled backward. EE reports that EE fell against the dozer blade. This employee complained of pain in hip and back. The EMT's on scene placed the patient on a backboard and EE was transported to the Hospital by ambulance.
