The employee went to the company clinic on 4/6/07 and was placed on restricted duty. The employee missed footing platform while stepping from one VMC loader to another, causing him to slip down several steps while holding onto the handrails, which resulted in a laceration to L-knee.

The employee was walking from the employee building to the shop on relatively flat ground covered with base material. The employee stumbled and fell. The employee's right ankle was fractured along with fractures to her left fibula. These injuries may be further explained by employee being diagnosed with osteoporosis.

Upon finishing maintenance and in the process of cleaning up the C-11 area, the employe finished wrapping up the torch hoses and welding leads. At ground level, he slipped landing on his right shoulder, unaware that ice was beneath the fresh snow presenting the slip fall hazard. The employee continued uninterupted work schedule until the restricted duty status of 5/15/07.

A Vulcan Contractor Services employee ID #01-03226 fractured his R fibula while working at Tuscumbia Quarry #01-00045. Employee threw a tie-down chain over a load to be hauled. The chain swung back under the load toward employee; he jumped back and feet became entangled in hoses attached to trailer causing him to fall.

The employee went to see the company doctor on 10/23/2006 and was taken off work by the doctor. While performing a pre-trip inspection, the employees R-leg fell between the left side track and push bar, resulting in a dislocated R-knee.

EE was cutting pins out of backstop to release belt after belt got hung up in head pulley. After the 3rd pin, the 4th pin broke, releasing backstop mount. Sudden movement caused EE to jump back and fall, striking his arm on step of head pulley, #2 conveyor.

Employee tried to break his fall and struck his left elbow on the loader bucket in the process. Employee continued working for another hour or so before realizing that the injury was going to need to be looked at.

The employee was diagnosed with a foot strain on 4/27/06, and at his reexam was placed on restrictions on 5/2/06.

Diagnosis - Foot sprain and ankle sprain. Originally reported as first aid only on 12/20/2005 - turned into medical treatment on 12/27/2005.

Employee was climbing 4-foot step ladder to gain access to D10 dozer in order to fuel it. When the employee reached the 3rd step, the ladder started to wobble and the employee lost his balance and fell. This resulted in a compound fracture of the right lower leg.
