On 6/15/23 an EE was replacing a ripper tooth on a CAT dozer. The EE used a sledgehammer to dislodge the ripper tooth and upon impact a roughly 1/4-1/2"" piece of the tooth broke off and struck the EE in the left calf. On 6/16/23 EE was taken to the clinic where an x-ray confirmed the object was lodged in their calf. EE was prescribed antibiotics.

EE was removing the segments on 988G loader bucket. EE was using 3/4"" air impact gun to remove nuts from plow bolts. The bolt loosened, descended allowing it to spin. EE used palm to push the bolt back to lock it in place to allow impact to remove the nut from top. The plow bolt engaged EE's glove pulling EE's hand into it and caused a severe cut across the right palm/thumb.

Employee was mixing backing epoxy material for installing crusher liners. The employee's jeans came in contact with a small amount of the backing that then soaked through to skin. Employee had skin irritation localized to this area, but also began experiencing rash-like irritation across other parts of body.

Emp ran a front end loader 10/24-27/16. 10/28, mgmt saw emp walking differently. Emp told mgmt back was sore running loader all wk. Said woke up Fri with pain in right lower back, down EE's right leg. Mgmt discussed first aid (rest, Ice) with emp and offered to take rest of day off and spend wknd resting. 10/31, emp continued first-aid to see if improves. 11/01 emp went to doc

Mechanic struck end of tooth with a hammer while replacing bucket tooth on 988G loader and piece of steel broke off tooth and hit him in the left eye.
Originally reported as First Aid Only - while vacuuming out electrical panel felt a possible bug bite, however employee was not sure it was a bug bite. Employee came in next day with left hand red, swollen and could barely bend or straighten fingers. Diagnosis - left hand infection.

Employee was climbing ladder to inspect jaw crusher, carrying a bucket of grease, when he slipped and fell approx. five (5) feet, landing on the leg and the ankle hit a rock causing the ankle to twist inward. Diagnosis - Foot sprain and ankle sprain. Originally reported as first aid only on 12/20/2005 - turned into medical treatment on 12/27/2005.
Employee slipped on first step of 988G Loader, causing loss of footing - overcorrected with left arm, pulling muscle(s). Diagnosis - Rotator Cuff Strain. Became medical treatment on 12/29/2005.
Running crusher plant while lime silo was being emptied with hydrated lime. Five (5) days later, employee informed office of ""rash or infection"" to feet. Employee stated that he thought it was from the lime.
