The employee was dismantling a 5' crusher. He was standing on a crossbeam and lost his balance and fell approximately 10' to a concrete pad below. He hit his head and cracked his hard hat.

Employee, who was using the water truck to water the road in the pit area, was washing spillage from underneath the 7'Crusher pedestal when he needed more hose to complete the task. He yanked on the hose causing his feet to slip in the mud, falling onto the frame of the crusher pedestal.

EE WAS UNDERNEATH THE PRIMARY CRUSHER AND HE WAS CLIMBING UP A LADDER CARRYING A BUCKET OF WATER. HE WAS ALMOST TO THE TOP AND HIS FOOT SLIPPED OUT FROM UNDER HIM AND HE FELL ABOUT 8' AND HIT A CEMENT BASE OF THE PLANT.
