On 02/20/2026 the injured employee was descending down the steps of a Komatsu 475 Wheeled Loader when they slipped and fell injuring their right knee. On 03/25/2026 MRI results revealed they had ruptured their right patellar tendon and was placed on work restrictions.
Tripped on pallet and EE kicked self in the calf

Fall was from approximately 8 feet resulting in injury. Time of accident was approximately 11:00 a.m. ambulance arrived approximately 11:20 a.m..

He sought medical treatment on the same day and was released to regular duty. 10/7/15, we find that he went to the clinic again and was given restricted duty w/o the knowledge or approval of his case manager. Still has not contacted CM.
Rigger failed to pay attention to details of task. When trailer attachment moved, the employee slipped and fell. Falling 9' shock absorber deployed. The employee did not hit the ground or any other object. Employee was bruised and sore.
Employee was walking out of the warehouse when he slipped on the ice hurting his left hip, back, and right elbow. He didn't seek treatment until 2/25/15, where he was placed on restricted duty.

First reportable treatment date is 4/25/12 in which he received an injection for a knee strain he did when he stepped in a shallow hole and twisted his knee. He hasn't had anything other than "first aid" until now.
He was cleaning the mirror with his hand on the handrail on the Cat truck. The handrail came loose and he fell to the ground. No medical treatment at that time. He decided for medical treatment on 5/20/12 and was released from care with the understanding that if he cont'd to have problems they would need to scope his knee. He had surgery 11/1/12.
