Fell from top of staircase of the crusher control house. Did three rolls down the steel staircase scraping both knees, bruising back and neck. No stitches required, employee released by doctor to regular duties as a supervisor.

Pouring concrete into an above ground form. 12 inches off the ground leaning on the mixer chute, when it moved he fell down on some form iron supports and lacerated his left lower leg. Employee received steri-strips to close and help heal wound.

Attempting to reach top of a steel plate to attach clamp, in effort to reach higher on plate he climbed up on an ecology block retaining wall & lost balance falling over the wall 80" to ground. The wall was 1 block high on the working side 3 blocks on the wall side. Wall was designed to interlock not flat on top & not intended to be used as work platform or walkway.

Worker was climbing out of classifier tank, slipped and fell. Worker hit head on hand rail and got right leg stuck between hand rail and tank, injuring knee.
