Showing all 29Contract Highway Truck driver fell while exiting cab of truck. Fell to the ground complained of back pain contacted EMS for transport to Clinch River Medical in Richlands Va.
The weather was bad, snow, ice etc. Employee pulled up to gates and got out of the vehicle. EE moved the gate arm to the left side of the road, turned to return to vehicle and fell onto right side.
Jones Petroleum, employee stepped off lift gate of EE's delivery and fell to ground. Landed on EE's left side, complained of pain to shoulder and ribs. Contacted EMS for assistance. The employee of JONES Petroleum refused to go to emergency room. Returned to truck and continued EE's work.
Descending stairway - hyperextended knee, went to the ground in sharp pain in left knee. Pain went away, knee sore went to hospital.
Employee was walking from MCC-1 aggregate primary to aggregate primary control tower and fell in mud and landed on left shoulder.

EE was assisting in the repair of a brace on the left side rear view mirror. EE had positioned himself in front of the operator's cab, using a pry bar to apply pressure on the mirror he lost his balance on the bar, slipped, causing him to fall approx 9' to the ground.
Was getting out of water truck, left foot slippd on steps letting him fall and hit right side of body on rocker panel of truck, sore under arm around ribs.
WAS WALKING DOWN CATWALK ON #68-BELT. LOST FOOTING FROM ICED CONDITION. FELL DOWN ON RIGHT SIDE ONTO CATWALK. RIGHT ARM GETTING NUMB AND TINGLING IN NECK ON RIGHT SIDE.
MECHANIC WAS PREPARING TO WELD PIN LOCK ON BOOM OF EXCAVATOR WHEN WELDER MISFIRED. EMPLOYEE TURNED TO GET AWAY FROM SMOKE & HE TRIPPED OVER A GASOLINE CAN & FELL TO THE GROUND ON HIS LEFT SHOULDER.
Was climbing off truck bed; had been putting block in bed to spread with. Fell off top step of side of bed, hitting on back and head.
WALKED DOWN GABION BELT CATWALK TO UNSTOP GABION CHUTE. PICKED UP 8-LB. SLEDGE HAMMER WITH RIGHT HAND. ATTEMPTED TO STEP FORWARD AND DOWN TO METAL BOX LOCATED AT BOTTOM OF GABION CHUTE AND ONTO THE GABION BOX LEDGE WHICH IS APPROX. 12 INCHES IN WIDTH. STEPPED DOWN APPROX. 8-10 INCHES ONTO THE BOX LEDGE - LEFT FOOT CURLED AND TWISTED ANKLE.
EE WAS CLEANING CHUTES WHEN HIS FOOT SLIPPED. HE STRADDLED THE HANDRAIL AND SIDE OF GUARD AND FELL DOWN ABOUT 4 FEET.
EMPLOYEE WAS STANDING ON CATWALK ATTEMPTING TO CUT A CHUTE LOOSE WHEN THE CATWALK HE WAS STANDING ON FELL. EMPLOYEE SUFFERED BACK, PELVIC, ANKLE AND LEG INJURIES.
STEPPING DOWN OUT OF TRUCK LEFT FOOT SLIPPED FELL AND ALL OF BODY WEIGHT WENT ON LEFT SIDE OF BACK.
SLIPPED ON ICE.
