Showing all 21Employee finished unloading material. He went to rack his hoses on his truck. As he was racking them, he slipped on mud and lost his footing causing him to twist his body to try and catch himself from falling. He ended spraining a muscle in his lower back.
Employee was standing on a six foot ladder. He over extended to reach out and fell over hurting his left arm and wrist.
Employee was walking and tripped on a curb at a local convenience store after refueling his company vehicle. He fell on his elbows and knees bruising them and injuring his right shoulder in the process.

However, his right foot and leg were on the ground and his left leg was still on the channel iron. At this time employee felt pain in his left knee.

Employee slipped and fell on a wet concrete surface, landing on his left hip. On 1/16 He reported "a knot about the size of a quarter" to the left hip area and tenderness to the touch. He continued to work while self medicating. On 1/21 he reported continued discomfort and that the knot had not subsided. At that time, he went to see a physician.

Employee was climbing down from cab of Volvo L70E front end loader. He had both hands on grab rails of ladder, his right foot was on bottom step, he lowered his left foot onto the ground. Upon placing weight on his left foot, employee said his knee twisted causing pain. Diagnosis from medical doctor describes injury as hamstring muscle strain.
EE WAS DISMOUNTING FROM THE FRONT END LOADER WHEN HE STEPPED ON A ROCK TWISTING HIS ANKLE. THIS RESULTED IN A SPRAINER LEFT ANKLE. HE MISSED THE BOTTOM STEP AS HE WAS DISMOUNTING.
EE WAS DRAGGING A CHAIN INTO THE MILL BUILDING. HE BUMPED A CAN OF PAINT AND IT SPILLED CAUSING HIM TO SLIP AND STRIKE HIS HEAD AGAINST AN OIL DRUM.
VICTIM SLIPPED AND FELL WHILE RODDING MILL FEED.CAUGHT ARM ON UNISTRUT.
VICTIM WAS TAKING MEASURMENTS FOR REPLACING THE HEAD PULLEY ASSEMBLY ON THE APRON FEEDER. AS HE DESCENDED THE LADDER HE LOST HIS BALANCE & FELL BACKWARD ONTO THE GROUND (APPROX. 5.5 FEET).
VICTIM CLIMBED ONTO PIPE TO CLEAR ACTERIAL FORM THE FEED CHUTE. VICTIM SLIPPED AND FELL APPROX. 9 1/2 FT TO THE FLOOR, LANDING ON THIS SIDE. VICTIM RETURNED TO WORK FOR HIS NEXT SHIFT, HOWEVER ON 9-12-01. HE REPORTED THAT HE COULD NOT COME TO WORK AND WAS INSTRUCTED TO SEE A DAY.
