STOCKPILING ORE AT MILL SITE STOCKPILE AREA. DUMP TRUCK DUMPED HIS LOAD ON STOCKPILE AND BEGAN TO PULL AWAY FROM PILE. WHILE SLOWING PULLING FORWARD AND BEGGINING TO TURN TO THE RIGHT, THE HAU L TRUCK RAN OVER A DOZER OPERATOR WHO WAS IN FRONT OF DUMP TRUCK THE INJURY WAS FATAL.

EE was dragging 4"" bulk hose across barge deck to tank for hook up. Upon getting close to tank, EE turned to hook up hose, striking the fill line with upper center part of forehead along hair line

The task was to connect the rigging to from the trackhoe bucket to the payloader. As the employee was connecting said rigging, the trackhoe bucket contacted the employees left shoulder and face.

Employee was installing a valve handle when attempting to climb down the ladder, lost his footing and fell onto the valve that was on the ground causing a laceration to his left hand.

Employee drove front end loader over a pot hole, throwing him against the windshield. Employee was not properly wearing his seat belt.

Employee complained that he had something in his eye after he had left work. He had been grinding metal earlier in his shift and may have gotten a metal particle in his eye when he wiped his face with his sleeve.

Employee tripped over boot cleaner outside of front door to the office and twisted her ankle.

Struck by flying object

Employee was opening one latch on a camlock pipe cap from a pressurized vessel. After releasing the one latch employee observed that the tank still had pressure. He closed a valve above the cap and then removed the other latch. The line blew the cap off striking employee in the right forearm. Diagnosis, contusion right forearm and corneal abrasion to right eye from dust.

EE was utilizing a pry bar through an inspection hole on a rock hopper trying to free a jammed rock. When the rock came free, the rock fell against the pry bar in the inspection hole. EE still had his hands around the pry bar. EE's finger was lacerated when the pry bar moved up and into a steel support for the hopper.

Employee was climbing down from cab of Volvo L70E front end loader. He had both hands on grab rails of ladder, his right foot was on bottom step, he lowered his left foot onto the ground. Upon placing weight on his left foot, employee said his knee twisted causing pain. Diagnosis from medical doctor describes injury as hamstring muscle strain.
