Employee was on a safe rack when the flooring gave way. Employee did not fall all the way through and fall protection was in use. During the process of catching self the employee bruised the left arm and left leg.
Employee fell from a chair roughly 56" inches from the ground. On impact, majority of body wait landed on the the left elbow causing a sprain.
3rd shift loadout operator struck right shin on gate while exiting the top of a hopper car from the ramp. Operator slipped on the ramp causing the fall. Right shin contusion diagnosis on 4/18/25.
Miner was dismounting from passenger side of a tractor trailer. Miner turned to climb down and missed grabbing the handle on the truck. This caused Miner to lose footing and fall to the ground. Miner struck knee on the ground in the fall, causing the tendon to separate from the kneecap.
A packaging operator was standing on the platform at the back of an empty slurry railcar when EE lost EE's footing, hitting EE's right arm on the break wheel of the railcar. The operator sustained bruising to the upper part of the right arm. The operator was prescribed antibiotics and a Tetanus shot.
Employee had injured knee a month and a half prior falling from first step of a ladder, but thought it was minor and would go away. When pain persisted, EE was sent to doctor for evaluation. Diagnosis was knee strain.
Employee arrived at work and was getting out of EE's truck to go clock in. As EE was getting out of EE's truck, EE's left foot slipped on black ice, and EE fell out of the truck onto the asphalt injuring left shoulder. It had snowed during the night and the parking lot was covered with black ice and snow.
The employee was turning to leave the wet lab and strained right knee
Electrician was descending the stairway. EE had one hand on the handrail, but was carrying a pipe in the other hand. EE missed the last step and as EE stepped down EE injured EE's right foot and ankle. EE heard a loud popping sound
After checking fluid levels, an employee twisted their knee when leaving the work area due to their foot being wedged between a pump and some piping.
Employee was stepping down from the excavator, EE's foot slipped, and EE hit EE's right knee on the excavator tracks.
Employee was pushed and as EE stepped back, EE stepped down missing a step, lost EE's balance and fell on EE's right knee sustaining injury to multiple body parts.
Hose for test procedure stretched across walkway. Employee tripped on hose and impacted EE's elbow.
Tripped on grating EE was installing and fell onto left wrist, resulting in a sprained wrist.
Employee slipped on ground and turned ankle. Restricted duty only for 48 hours
Miner stacked pallets 20 high for use in packaging supersacks. When they reached for the top one a foot got caught under the pallet stack and when they went to step back was unable to. They fell back on to the roller conveyor and sustained a back contusion and sprain of ligaments of thoracic spine.
Employee was walking down stars and twisted right knee
Securing super sack on spout. Cord snapped and employee lost balance and fell into beam.
Manager tripped and fell and injured ankle on 6/21. EE reported as a near miss, thinking the injury minor. After a three weeks and the ankle did not feel any better, ee went for medical care. Doctor diagnosed strained Achilles tendon and put ee on light duty.
Employee slipped and fell against pallet handling device, bruising their forearm.
Miner gaining access to the driver's cab of a water truck, missed the handrail bar under the front window, causing a fall.
Employee was exiting 522 Haul Truck using ladder. The employee missed the last step and grabbed the ladder with left arm to prevent a fall. Straining left shoulder.
Employee was crossing RR track, stepped over one rail and tripped on second rail, falling and landing on lower back. Employee seen at ER and released with no restrictions. On 10/3/2016, employee still in pain so was evaluated again at doctor, who prescribed light duty.
Employee was removing the plywood bed on electric cart when they tripped on a tripod pipe stand leg, stumbled and hit the maintenance shop roll up door roller guide frame causing a laceration to right arm.
Employee was dismounting from a haul truck chassis structure 7 ft high onto a 4 ft ladder when he fell onto the wash bay concrete floor.
Employee was stepping down from an elevated platform to retrieve a hose. His left foot sank into mud/fines, causing him to pitch forward and fall to the ground, spraining his left ankle in the process.
Stepped down from rear bumper of pickup truck to ground and left knee popped
Employee was prying a fuel pickup on a Bobcat, when the pickup dislodge employee struck the Bobcat's exhaust pipe with his front teeth. Dentist concluded a filling was lost.
Individual entered the rear of a one ton rack body truck containing a steel frame leaning against the East side of the truck rack. Individual was previously advised and trained not to do this. No damage to the truck or frame. Individual sustained a compound fracture to the lower leg.
Employee fell into wall behind him onto right shoulder.
employees hand slipped off the hand rail of his truck while he was climbing into it, it was raining and the handle was slippery.
Slipped on ice falling onto right shoulder.
Employee was replacing skirting brackets on a conveyor belt when he tripped, stumbled, and stepped hard on his right foot. Employee was placed on restricted duty.
Employee was walking in the mill bldg, 2nd floor, when he lost his balance and stumbled to the floor on the same level hitting his left knee against equipment causing an open wound.
Employee was washing a concrete pad with a fire hose, he slipped, causing a twisting motion to the low back.
Employee stepped off walkway onto grating and turned ankle. This was just one step to next platform.
Employee was assisting two other employees move a junction box. Employee slipped and fell using his left hand to brace his fall. Junction box landed on his left pinky finger.
On 9-27-2010 employee submitted an accident report claiming that on 4-19-2010 his foot got tangled on banding material stuck under a pallet causing him to trip and fall off a platform and hit a hopper before landing on the floor. Accident was reported 23 weeks after the injury. Employee went on FMLA on 5-12-2010.
Employee slipped from the footstep while mounting a forklift and struck his abdomen on the chair rail causing a contusion.
While walking under ball mill, employee slipped on media and fell onto his back causing a strain to the lower lumbar area.

Employee was walking up the mill building stairs when he hit a stair step edge with his boot causing him to fall forward. When falling he hit handrail support post between 4th & 5th fingers on his left hand causing a laceration.
Employee tripped on grating and fell chest first against a concrete containment wall causing a chest wall contusion
Employee was directing the loading of an excavator bucket into a haul truck bed when he fell from the truck and landed on both feet.

Employee was climbing down platform at end of railcar and missed the last step, striking his right side on the railcar and causing a bruise. 3 days light duty.
Fell onto right wrist while loading a boxcar of product causing a severe sprain.

Employee was cutting stainless plate with a carbon arc torch while standing on scaffolding. He noticed that the protective tarp was starting to smolder and catch on fire. He jumped down the 3 feet to the ground to put out the tarp. While doing so, he landed badly and hurt his knee.
