Employee states that while getting ice cubes out of the ice machine in Plt 2 lunchroom he noticed the ice bucket was overflowing so he dumped some ice out of the bucket. In doing so some ice fell out of the bucket onto the floor. EE then stepped on the ice and his feet went out from under him. he landed full weight on his shoulder.
