Showing all 6Employee slipped on some wet material under the conveyor in the pit in July 2013. He felt fine after the incident and did not report it to the company. The employee started to have some discomfort with his left knee in early August 2013 he was place on work modifications after an MRI on 8/12/2013. with a sprained ACL/PCL Physical Therapy for a treatment plan.
The miner was refueling the EL-Jay. As he stepped off of the ladder onto the ground he rolled his left ankle.
An employee slipped and hurt himself while standing on a wet conveyor belt. The frost had come off of the belt but it was still damp and slippery.
Employee was cleaning the loader windows and lost his balance as he moved across the walk way on the loader, causing him to fall. He landed on his right hip when he hit the ground.
The employee was getting on and off a dozer to check each truck. He didn't take the proper time required to clean the mud off of the steps. He stepped off the last step and slipped, cutting his right shin (this was on the 18th, basic first aid was done by the employee). After five days he suspected an infection, we sent him in to check it out, and antibiotics were admistered.
