Employee was cleaning an air filter and got dust in EE's eye.

The miner was refueling the EL-Jay. As he stepped off of the ladder onto the ground he rolled his left ankle.

Employee was bent over working to change a roll on a conveyor for about 2 minutes. When he stood up he experienced muscle spasms.

Fall from machine

Employee was cleaning the loader windows and lost his balance as he moved across the walk way on the loader, causing him to fall. He landed on his right hip when he hit the ground.

Fall onto or against objects

The employee was getting on and off a dozer to check each truck. He didn't take the proper time required to clean the mud off of the steps. He stepped off the last step and slipped, cutting his right shin (this was on the 18th, basic first aid was done by the employee). After five days he suspected an infection, we sent him in to check it out, and antibiotics were admistered.

Struck against a moving object

While a haul truck was going up an icy hill, the truck's transmission shifted down, resulting in tires losing traction - the truck slid backwards down the hill and stopped suddenly at the bottom - the quick stop made the driver's head strike the cab - first aid was given, he went to a clinic & received 6 stitches - he was discharged with no restrictions - back to work after lunch.
