Checking sheeting on roof to see if placed right to screw down, there was condensation on roof sheeting and EE started sliding off the roof and landed on EE's feet.

Originally went to ER. Was released to return to work. After follow-up with ortho DR. was placed on restricted duty on 11/06/24. EE was walking to their work truck when they tripped on some uneven concrete and fell. Injuring their right shoulder.

Miner stepped out of operator's station of Cat 14H blade to close open engine bay door. In doing so miner stepped onto rear tire that was covered in mud, slipping & falling to the ground. This resulted in spraining left wrist. Miner had MRI completed on 3/10/24 & results were provided to SRMG safety on 3/12/24 stating there was in fact a fracture in the wrist. No other issues.

EE was walking routine for Preventative maintenance. EE came down a flight of stairs. At the bottom, EE turned to the right and started walking. EE stepped on an uneven joint in the concrete and twisted EE's left ankle.

EE was inspecting RM-306(raw mill) roller deck. EE tripped on a support bracket for the lubrication system lines. The tripping motion caused the EE to fall forward into the rocker arm assembly, causing EE's right arm to strain backwards causing shoulder pain. Originally reported as a first aid, but due to continued discomfort medical attention was sought on 5/10/23.

Miner was running for cover from the starting rain storm when EE stepped on the concrete near the flocculent tank. In doing so EE stepped on a small amount of spilled wet flock and slipped. Miner said they felt a pain in lower back.

On the morning of 12/6/2022 miner reported to their supervisor that on 12/05/2022 they had jumped from the Jaw crusher frame rail and injured their right knee upon landing on uneven surface. Employee was sent for medical and placed on restricted duty for a sprain knee.

Miner was stepping out of service truck when they missed the bottom step and fell to the ground. Miner impacted the ground with left shoulder resulting in an injury to the shoulder.

Miner was inspecting the head pully of a conveyor belt on the Jaw crusher while standing on the chassis of the jaw. Miner stepped over to one of the tires and it rotated causing miner to start to fall. Miner grabbed the support leg of the conveyer to stop the fall and strained right shoulder and upper arm. Miner was placed on restricted duty by medical.

Operator was dismounting the 670 G Excavator. Using 3-points of contact, operator's right ankle rolled when placing foot on uneven ground resulting in a sprained right ankle.

Miner was climbing down out of the 980K loader during the pre-trip inspection and missed the bottom rung of the ladder that is attached to the loader. This resulted in EE falling and landing on hands and back. 6 hours later the miner requested to be checked by medical. X-rays came back negative and miner was given one off work for recovery by the doctor.

Employee tripped over material discharge hose while operating gunite machine, landing on left wrist.

Miner was climbing out of the cab on a 621 G water pull when EE missed one of the steps on the way down. This resulted in the miner falling from the equipment landing on EE's backside and causing a contusion to EE's mid and lower back. Miner was placed on restricted duty by the urgent care center.

Driver was cleaning side window did not have three points of contact when stepping off the semi. Twisted and broke right ankle on flat surface.

Team loaded excavator onto trailer for transport. Employee climbed to top of equipment to get a height measurement for permitting. EE was not wearing fall arrest gear. While climbing back down, EE slipped on some hydraulic fluid and fell approx. 16' to the ground. EE's left arm hit the trailer beam, EE bounced off the track and landed on the ground.

Miner was climbing into a IT-28 wheel loader and missed step. As employee started to fall they caught themselves with right arm and felt a pop straining right shoulder. No medical treatment was requested at this time and a report was made. On 4/11/17 miner requested to go to the doctors and per doctors request was placed on restricted duty on 4/11/2017.

EE was exiting the quarry office building and fell onto the platform/steps outside the doors. EE cut EE's right shin and was sent for medical evaluation. EE received 5 stitches to close the wound and was returned to duty without restrictions.

EE was pulling a grease line to the Primary Crusher when he missed a step and reported straining his neck while catching his balance. EE was taken to the Dr., diagnosed with a neck muscle spasm and released to work with restrictions.

Good weather; Employee was standing on the back of his truck and went to step down from truck and apparently his shoe got caught on something causing him to fall to ground. He was not more than two feet above ground when he fell. He claims that he injured his shoulder, arm and knee.

EE was coming down the stairs from the second level on R-202 and reported losing his footing and feeling a "pop" in his right shoulder as he held on with his right hand to steady himself. EE reported as a First Aid Only case on 06/21/14 and did not seek medical evaluation until 06/27/14. He was diagnosed with a "right shoulder strain" and released to work with restrictions.

Walking on platform and slipped on klinker and fell forward twisting ankle.

EE reported falling on 29Mar13 but did not seek medical attention until 22Apr13. EE was opening a door to a storage shed when wasps flew towards him. As he was stepping backwards to get away from the wasps, he tripped over some pallets and fell to the ground. EE was diagnosed on 22Apr13 with a contusion to the right shoulder and released to work with restrictions.

EE was walking down the stairs of the 992G loader while carrying a bucket and slipped from one step to the next step down. The steps were wet due to rain and his heel slipped from the edge of the traction control on one step to the next step down. (8"drop) He was using the handrail and did not fall. EE was diagnosed with a back strain and had work restrictions for 1 week.

EE was lubricating a tail pulley and reported slipping in mud created by recent rain storms and twisting his left knee. EE reported the incident to his supervisor the following day 9/29/11 as a first aid only and did not request medical attention until 10/05/11. EE was diagnosed with a "strain" to the left knee and released to work with restrictions.

EE was exiting the cab of the skid steer and felt a "pop" and sharp pain in his back as he stepped down to ground level. EE was sent to the company physician for examination and diagnosed with a "strained" back muscle. EE returned to work the same day with minor restrictions.

As he stepped off a loader backwards and turned to walk away he felt a slight tingle which gradually got worse.

EE slipped and lost his balance as he was exiting the prep room and twisted/strained his back while trying to regain his balance.

Tacking rail in place and twisted knee moving to tack other side of railing.

Walking down the stairs he twisted his right knee.

He stumbled on large rocks and fell back on to a rock berm.

Missed the step and his right leg went in between the steps causing him to fall backwards putting pressure on his leg.

As he went to return the fuel hose to the stand, he twisted his ankle and fell.

As he dismounted the haul truck he felt a pull in the groin area.

He was walking away from the trash can at the wash plant after throwing away some trash and slipped on the mud falling on his right hand.

He was putting fuel into the manlift, and as he began to climb on from the forward side his foot slipped off the frame causing him to fall and landed on his right knee.

He was checking the under cone, and then jumped down off the conveyor belt which was 3 feet on to a dirt pile causing injury to his neck.

As he went to step up on the gen set platform, he lost his footing and fell. When he fell he put his arm out to catch himself.

As he was pulling on a pry bar at the welding shop table the bar slipped causing him to fall.

As he was walking down the stairs at the feed end of the kiln onto the ground level there is a 2" drop from one concrete slab onto another and as he stepped off he twisted his left ankle.

He was standing on a tram, installing a new hydr. hose in the articulating area of the machine and he lost his footing and fell to the ground.

HE WAS CLIMBING ON THE DRILL AND HIS FOOT SLIPPED, AND AS HE CAUGHT HIMSELF, HE PUT ALL HIS WEIGHT ON HIS RIGHT ARM AND HE FELT A SHARP RIPPING PAIN IN HIS SHOULDER.

THE FLOOR WAS SLICK AND AS SHE WENT TO STEP OUT OF HER OFFICE SHE SLIPPED BACKWARDS AND FELL AGAINST THE WALL AND TO THE FLOOR HITTING HER LEFT SIDE. THIS EMPLOYEE DID NOT SEEK ANY MEDICAL ATTENTION UNTIL 08/11/04

AS HE WAS CLIMBING OFF THE WATER POOL HE STEPPED ON A ROCK, CAUSING HIM TO FALL AND TWISTING HIS ANKLE.

"RAIN & MUD CONDITION" EE WAS RETURNING A TROUGHING ROLLER SET TO THE BACK OF THE MAINTENANCE TRUCK. HE THEN BACKED AWAY FROM THE TRUCK AND SLIPPED IN THE MUD. WHEN HE STARTED TO SLIP, HE TRIED TO CATCH HIS BALANCE. HE THEN FELT AND HEARD HIS BACK POP AND INSTANTLY HAD LOWER BACK PAIN.

WALKING THRU THE MECHANIC SHOP SHE TRIPPED AND LOST HER FOOTING DUE TO A DAMAGED FLOOR CAUSING HER TO FALL ON HER RIGHT WRIST.

AS HE WAS PULLING ON THE V-BELT TO TIGHTEN THE WIPER, THE V-BELT BROKE. THE EE FELL BACKWARDS AGAINST THE HEAD PULLEY INJURING HIS BACK.

AS HE WAS WALKING HE STEPPED ON A PLATE AND HIS FEET SLIPPED OUT FROM UNDER HIM CAUSING HIM TO FALL AND LANDING IN A SITTING POSITION WITH HIS HANDS BEHIND HIM.

AS SHE WALKED UP TO THE TIME CLOCK THE TOE OF HER BOOT CAUGHT ON THE EDGE OF THE SIDEWALK CAUSING HER TO FALL AND INJURE HER SHOULDER.

WALKING DOWN STEPS WITH BOXES IN HAND, SHE MISSED A STEP, FELL INJURING RIGHT HIP. SHE WAS TREATED & RELEASED WITH NO RESTRICTIONS. APPROX 3 MONTHS LATER SHE HAD PAIN IN LEFT HIP. HER PHYCIAN SAID IT WAS FROM PREVIOUS INJURY. AFTER RE-EVALUATION OUR DR PUT HER ON RESTRICTICTIVE DUTY. ON 7/1/03 THIS ACCIDENT BECAME REPORTABLE.

AS HE WENT TO PICK UP AN IDLER ROLLER HE LOST HIS BALANCE AND FELL RIGHT ON TOP OF IT LANDING ON HIS SIDE.
