Showing all 20Miner/Loader Operator was dismounting a 1996 CAT 970 to pick up a cone, when his right leg banged against the loader platform, scraping his shin which he grabbed, losing his safety 3-point contact and causing him to fall from ladder steps and injuring his pelvis/hip. Miner suffered fracture to his pelvis.
Miner/Mant. shop, was climbing down from elevated position on gradall scaler, when he slipped, losing balance, sliding off fender skirt, approx. 4.5' to floor, bracing w/hands. Position was narrow, without elements of 3-point contact, and too low for fall protection device/rig. Inj. recordable because of wrist brace, miner rtn. to work.
Miner works with maint. and was assisting @ Line #2 loadout area, when a slippage occurred on the long belt feed line. Miner was working in cleaning up rock spillage when his left foot rolled on him, spraining his ankle. Injury considered minor to moderate. Miner still working.
Maint. Miner was standing on elevated screen cover, (with handrails present/fall protection), when right foot slipped and miner fell against handrail/mid rail, twisting right leg/knee. Miner did not seek medical treatment @ time. Miner sought treatment for sore knee on 19th, and is working (with brace) awaiting later MRI. Knee diag. as strain-sprain.
Miner was dis-mounting a ladder rung to step down to the floor @ the mill operations, when he stated his foot (L), slipped and he felt a "Popping" sensation followed by a stabbing pain. Miner was transported to company medical provider where an x-ray revealed a small fx above his left little toe. Miner was wearing safety boots.
While operating roller-mill equipment, EE was talking to coworker. EE stepped back & tripped over a pallet. EE stated he received a twisting type of discomfort to R-shoulder & R-hand. EE did not wish medical eval. at the time, but reported medical eval. request on 6-13-05. Sent to company doctor on 6/13/05 & placed on restricted duty.
Miner was doing ground contol & roof scaling in the area of pillar 2P103 on the R&S section of the working face, when some loose rock fell near him causing him to step forward suddenly to avoid being struck. He lost his balance and fell forward off a muckpile, causing him to flip head over a couple of times, striking his head and causing a small bump and cut.
EE WAS EXAMINING A ROOM WITH FELLOW SCALER. WHILE LOOKING OVERHEAD AT ROOF CONDITIONS, HE STEPPED INTO A HOLE IN THE FLOOR. HE STRUCK HIS LEFT LEG ON A ROCK PROTRUDING UP FROM THE FLOOR OF ROOM. HE SUFFERED A LACERATION WHICH REQUIRED 22 STITCHES. RELEASED BACK TO WORK WITH RESTRICTIONS.
HE SLIPPED OFF STEP OF TRUCK WHILE GETTING IN. SPRAIN/STRAIN TO BACK.
GROUND TESTING ON EQUIPMENT. TRIPPED ON CATWALK HITTING HEAD ON EDGE OF TROUGHING ROLLER.
CLIMBING DOWN STEPS OF LOADER HE MISSED THE BOTTOM STEP AND FELL.
DISMOUNTING FROM TRUCK - SLIPPED ON ICE.
EE WAS STEPPING DOWN OFF OF LOADER AND MISSED STEP, FALLING TO GROUND FLAT ON HIS BACK.
