EE was moving rock when he slipped on a water puddle and twisted his left ankle. He was sent to the doctor and instructed to stay off of his ankle until the swelling decreases.
Employee was carrying rock to the trash bin. He jumped on a metal step and fell through the step. He twisted his right knee as he fell. There was slight swellig and immediate pain. Upon dr. examination his knee was found to be strained. He was directed to stay off of it for a day then return to work.
EE was helping to hand load a bucket of rock on the load. He stepped onto the bulk pile and lost his footing. EE felt immediate pain in lower back as he fell into the rocks. He took ibuprofen @ 10:20 to relieve pain and possible swelling. Doctor's appointment was made for eval.
EE WAS COMING DOWN A LADDER FROM THE CAB OF A TRACTOR. THE LADDER WAS MUDDY AND MISSING THE BOTTOM RUNG. HIS FOOT SLIPPED AND HE LANDED TOE-FIRST ON HIS LEFT FOOT. HIS TOES COMPRESSED IN THE STEEL PROTECTED PART OF HIS BOOT. IMMEDIATE SWELLING AND PAIN. VISIBLE BRUISING. (BONE ON TOP OF FOOT FRACTURED.)
EMPLOYEE SLIPPED ON A DRAIN AND HIT HIS LEFT ELBOW ON THE CHOPPER TABLE. EMPLOYEE FELT A PAIN IN HIS NECK
Walking across grating, grating gave way & his leg went through.
EE STEPPED OFF LEDGE & SLIPPED, BECAUSE HIS BOOTS WERE WET. HE TRIED TO STEADY HIS FOOTING AND CATCH HIMSELF, BUT TWISTED (POSSIBLY BENT BACK) HIS FOOT. BIG TOE ON RIGHT FOOT WAS SWOLLEN & DISCOLORED.
EE WAS STANDING ON SLAB STICKING OUT FROM A PALLET TO ADJUST STONE AT TOP OF PALLET. SLAB GAVE WAY/BROKE BENEATH HIM; CAUSED HIM TO FALL AND SCRAPE LEFT LEG ON BROKEN PIECE OF SLAB ON HIS WAY TO THE GROUND.
