Employee had a seizure was not injured. We did call a ambulance and ambulance came and took EE to the ER in Abilene, Tx. Waiting to hear back on if it was due to prior health issues.

Crane Employee was unloading slab from cutting floor and was putting strap on stone slab when a slab that was around 6"" away had broke fell on the left ring finger of employee and broke tip of finger. The employee had hand by another stone slab putting strap on another slab when the one that broke fell over striking employee's finger.

Operator was stepping off of tractor at the end of the shift. And operator stepped to far back and heal partially caught on roughback strip causing employee to twist left ankle.

Employee was flipping a stone on roller table to push thru to chopper. When employee flipped stone he hit his right pinkie finger between stone and roller. Broke tip of pinkie and took chunk of pinkie out from under the finger nail.

Employee is a chopper Op. on this morning he was chopping stone. on the event. He had his hand on stone and went to bring head down to chop and the frame of the head of the chopper come down on his left hand and broke his left index finger and middle finger.

EE was unloading heal to put into trash bucket. The heal usually is 1""-3"" thick. The EE was trying to break heal when 2-3.5"" strips fell over on his right hand and thumb. Dr's note soft tissue bruise.

Dry cracks in the rock, was palleting stone and one had a dry crack and broke landing on his thumb and breaking thumb. EE was wearing gloves.

Rock fell over on top of his right foot.

Armando was trying to flip a hill. The hill fell backward and fell on his middle right finger. Finger was caught between hill and side rail.

Slipped in drain ditch, hit his left shoulder on a large rock.

Moving a rock to its side and pinched his left testical.

Employee was trying to move a rock on the flipper table. Started to fall and put his and on flipper table and twisted his left wrist.

Went to jump off his skid loader and his knee popped.

Employee was on Kawasaki loader when another employee found him slumped over the steering wheel. He wasn't responsive and CPR was started. Ambulance was called. EMS continued CPR and employee was pronounced dead at the hospital from a heart attack.

Employee was hammering rocks for bulk pile. He finished one rock, threw it in the bucket. As he reached for another rock, another employee threw another rock on the table. His pinkie finger was struck and sliced open at the first segment. He was taken to the ER and stitches were given. Employee was placed on light duty until doctor release.

Employee was pushing rock through chopper window when he smashed his finger between two rocks. After reporting to the office for first aid, he was sent to the Dr for stitches. Employee received 6 stitches and was released to return to work. He returned to the doctor in one week to remove stitches.

Employee was manually pushing rock on top of the saw table. His right ring finger was pinched between the wooden table and the rock. The tip of his finger was split open. First aid was given and determined a doctor should examine his finger. Upon doctor evaluation employee received 7 stitches.

Employee was moving rock from roller table and stacking it on pallets. his left hand was smashed between two rocks when a co worker prematurely pushed the next rock down the line. His left ring finger was broken. He had surgery 1/22/08 to insert a pin in his finger.

EE was moving rock when he slipped on a water puddle and twisted his left ankle. He was sent to the doctor and instructed to stay off of his ankle until the swelling decreases.

Employee was carrying rock to the trash bin. He jumped on a metal step and fell through the step. He twisted his right knee as he fell. There was slight swellig and immediate pain. Upon dr. examination his knee was found to be strained. He was directed to stay off of it for a day then return to work.

Employee was moving rock into a bucket when he heard and felt his back pop. There was immediate pain. When walking he feels a painful twitch. Employee was sent to the doctor for an exam.

Employee was lifting rocks and moving them to the opposite side of his saw. He felt a pull or strain across his chest. He awoke the following morning and could barely move. He reported the injury on 8/16/07. Upon his visit to the doctor, employee was diagnosed w/muscle pain and referred to his family doctor for follow up.

EE was stacking pallet, when another EE tossed a rock and smashed his finger. Left middle finger had a laceration on the finger tip. Treated with first aid and sent to doctor. Steristips applied and told to stay off work for one week.

Employee was working the window at the chopper. He was pushing rock through the window & placed his left hand on a roller. The roller was spinning and his left hand was pulled between rollers. Pulled his hand out, but middle finger was injured. Referred to specialist who cut bone back to 2nd knuckle of middle finger.

EE was helping to load rock onto table. The crane moved, causing the strap to catch his left hand. EE had a deep laceration to the pad of his palm. Immediate first aid was provided and taken to the ER for treatment. He received 12 stitches. On light duty until he sees a specialist.

EE was stacking a pallet, when a rock fell off and struck his right knee. There was immediate pain and slight swelling. He came to work the next evening and then worked his entire shift. His knee was extremely swollen and painful. He then called to report his injury on 6/28/07, was sent to clinic for knee strain and pain.

EE was helping to hand load a bucket of rock on the load. He stepped onto the bulk pile and lost his footing. EE felt immediate pain in lower back as he fell into the rocks. He took ibuprofen @ 10:20 to relieve pain and possible swelling. Doctor's appointment was made for eval.

Employee was stacking rocks when his hand got stuck between two rocks. After removing the top rock and releasing his hand, the tip of his right middle finger appeared to be broken, bent and unable to flex. Upon doctor's examination, it was found to be broken.

Employee was moving a rock w/crane. The rock shifted in the strap and the ee lost control due to momentum. His right leg was pinned between the rock and the wall of the building immediate swelling and bruisng upon dr. visit, he was diagnosed w/torn tendons in his right shin/calf.

Employee was helping to remove excess rock from the rolling table. The rock he was moving got stuck between rollers. As he was moving the rock, the strap became loose and the rock fell and struck him on the top of his left foot. Upon doctor's visit foot was found to be fractured.

Employee was reaching beside his seat while on his skid steer when he cut his left middle finger knuckle on a piece of metal under the seat. Upon doctor's visit, he received 2 stitches.

EE was cleaning up to go home when he started to feel pain and discomfort in R shoulder and upper middle back. He reported his possible injury to the office at 5:45 pm 8-9-06. Upon doctors visit he was advised to rest and return to work the following Monday. It was apulled muscle.

EE was stacking rock when another employee smashed his left thumb between another rock and the one he was placing.

Employee was moving a rock along the roller of the chopper when he caught his left pinkie between the rock and the roller. He cut his finger on the rock. Was taken to the ER and received 5 stitches.

Employee was turning a rock when he twisted his right hand. He felt an immediate pain. Upon the doctors visit, a fractured bone was found in the palm of his hand.

Employee was unloading a rock from the chopper table. The rock broke and a large piece fell on his left foot just behind his toes. Some swelling and bruising. Upon doctor's visit, there was no breakage.

Employee was pushing rock through chopper when he felt a pain on his left side under his rib cage. He was sent to the doctor where he was referred to another physician for followup for possible hernia or pulled muscle.

Employee was running a tractor at the quarry when another employee noticed his tractor lost control and was going to hit a structure. Upon inspection the employee was found to be unconscious. First aid and CPR was begun. Ambulance was called. Upon EMT arrival and work to revive, employee was pronounced dead.

Another employee was picking up a rock from the floor with skat track. The rock rolled off the forks and on to ee's left foot. Immediate swelling and pain. Was taken to ER given a shot and told to return for xray when swelling reduced.

ee was throwing rock into trash bin, when the rock broke & hit him in the middle of the back. Ee did not report the injuryuntil 4/25/06 at 1:30pm.

EE had been operating a skat track then got off to help stack a pallet. As he bent over to pick up a rock, he felt a pain in his lower middle back.

Employee was using a 2x4 to lift a rock and insert a strap. Rock fell off 2x4 and smashed his left hand. Immediate swelling and pain. Fingers were twitching.

Employee has moving rocks into a bucket. One of the rocks cracked on a roller. As the employee has picking up the cracked rock, it broke in half and fell on the top of his left foot. The rock hit just behind his steel toe. There was an abrasion and slight swelling.

Employee reported his injury on 11/4/05. His back had been hurting him for a while. He had not been wearing a back belt at work. After a few days he started to hear his back belt. The back pain persisted and he decided to report the injury and request a doctor;s appointment.

Employee was moving a rock on to loading table. When the rock fell off of the roller. The rock struck his right foot, on top behind the steel toe. Lots of swelling and immediate pain and bruising.

EMPLOYEE WAS PUTTING ROCK ON THE TABLE. HE SMASHED HIS LEFT PINKIE BETWEEN THE ROCK HE WAS CARRYING AND THE ROCK ALREADY ON THE TABLE. PINKIE WAS SWOLLEN AND BRUISED. THE FINGERNAIL WAS POPPED OUT.

Employee was working at the chopper table when a rock fell off the table. The rock hit his foot right behind the steel toe of the left foot.

EMPLOYEE WAS PUSHING BLOCK THROUGH THE CHOPPER WINDOW WHEN HE FELT A PULL IN HIS BACK. HE WAS WEARING A BACK SAFETY BELT.

Employee was cleaning chopper. He was holding up the table to clean underneath and the table slipped smashing his left hand. His middle finger and ring finger tips were severed. Upon hospitalization, the middle finger was re-attached.

Employee was working outside when he became extremely overheated. Employee poured an ice cold cup of water over his head. Within moments the employee fainted. He was transported to the office. He had trouble maintaining lucidity.

EE WAS FLIPPING A ROCK ON THE SAW TABLE WHEN HE SMASHED HIS RIGHT PINKY.

EMPLOYEE WAS UNLOADING TABLE WHEN A 4"" SLAB FELL AND STRUCK HIM ACROSS THE RIGHT SHIN. IMMEDIATE PAIN. ASSUMED BROKEN LEG.

EE WAS COMING DOWN A LADDER FROM THE CAB OF A TRACTOR. THE LADDER WAS MUDDY AND MISSING THE BOTTOM RUNG. HIS FOOT SLIPPED AND HE LANDED TOE-FIRST ON HIS LEFT FOOT. HIS TOES COMPRESSED IN THE STEEL PROTECTED PART OF HIS BOOT. IMMEDIATE SWELLING AND PAIN. VISIBLE BRUISING. (BONE ON TOP OF FOOT FRACTURED.)

EMPLOYEE WAS ATTEMPTING TO REPAIR A DAMAGED CONVEYOR ROOLER WHEN A ROLLER POPPED UP AND HIT HIM IN THE LEFT FOREARM

Employee was loading rocks onto the chopper table. As rock was placed on the roller, a rock slipped forward from the opposite end and pinched the left pinkie finger.

EMPLOYEE WAS WALKING BY THE CHOPPER TABLE AND A ROCK SLIPPED. THE ROCK STRUCK THE EMPLOYEE ON THE LEFT SHIN. THERE WAS A GASH WITH EXPOSED TISSUE

5 of my guys went to job site to finish crane rail hoist system. They were all working together. Brought basket into building without anyone checking basket. Went to work welding angle bracing in. 1 man was trying to hold & weld. Another man walked from 1 end of basket to other. Basket tipped & fell off forks. 1 man stepped off, 1 man rode basket down.

Employee was stacking rocks on a pallet and smashed his right index finger between two rocks. His finger was bruised and swollen.

EMPLOYEE SLIPPED ON A DRAIN AND HIT HIS LEFT ELBOW ON THE CHOPPER TABLE. EMPLOYEE FELT A PAIN IN HIS NECK

EMPLOYEE WAS PICKING UP ROCKS TO LOAD ON A PALLET. ROCKS BEGAN TO SLIP AND THE EMPLOYEE ATTEMPTED TO STOP THEM. HIS RIGHT WRIST BENT BACK. THERE WAS IMMEDIATE PAIN AND SWELLING.

Employee was pushing a rock through the chopper window when it tipped over and smashed his right ring finger.

Employee was pushing a rock on the table when another rock smashed his right thumb.

EMPLOYEE WAS WAITING FOR ROCK TO BE UNLOADED FROM SCAT TRACK. SCAT TRACK BUCKET FELL AND ROCK TUMBLED OUT. ROCK STRUCK EMPLOYEE ON LEFT FOOT. IMMEDIATE PAIN AND SWELLING IN FOOT AND ANKLE.

EE WAS FLIPPING A ROCK AT THE CHOPPER, EE CAUGHT RIGHT INDEX FINGER BETWEEN THE ROCK AND THE STEEL ROLLER. EE RECEIVED 6 STITCHES BECAUSE OF EXPOSED FLESH

Employee was loading a table. The rock he was loading fell. A broken piece struck the employee on his right foot - on the bridge.

EMPLOYEE WAS PICKING UP TRASH ROCK TO THROW IN A BUCKET WHEN HE TWISTED HIS LEFT KNEE. HE FELT AN INTERNAL BURNING SENSATION AND KNEE SWELLED

EMPLOYEE WAS PITCHING A ROCK WHEN IT BROKE IN HALF AND FELL ON THE TOP PART OF HIS LEFT FOOT.

EE WAS FLIPPING A ROCK ON THE TABLE. THE ROCK HAD A DRYCRACK IN IT AND BROKE INTO 2 PIECES. ONE PIECE FELL ON HIS LEFT FOOT. HIS FOOT IMMEDIATELY SWELLED & BECAME DIFFICULT TO WITHSTAND WEIGHT OR PRESSURE.

Ee was in the process of edging a rock. as he was pulling the strap to move the rock, the rock broke and fell. as the rock was falling, it grazed the ee's right wrist. he experienced some swelling and tenderness. x-rays revealed no fracture or broken bones. ee sustained a superficial scrape on his inner right wrist.

EE WAS SLIDING 2 X 4'S UNDER A ROCK IN PREPARATION TO INSERT FORKS. THE ROCK BROKE AND SMASHED HIS LEFT HAND BETWEEN IT AND THE 2 X 4'S. LEFT HAND WAS SWOLLEN AND BRUISED. EE WAS UNABLE TO MOVE LEFT HAND.

EMPLOYEE WAS LIFTING A ROCK WITH HOIST. HE TWISTED BACK WHEN SETTING IT DOWN. MIDDLE TO UPPER BACK HURTS.

Employee was using crane (overhead) to lift rock to prep for cutting; he placed a steel bar under one of the straps to help get it around the rock; the bar slipped and hit him in the ribs.

EMPLOYEE WAS MOVING A ROCK ALONG THE CONVEYOR AND PULLED TO TURN IT; HE FELT SOMETHING PULL NEAR HIS LEFT UPPER STOMACH AREA AND HAD TROUBLE BREATHING.

EMPLOYEE WAS LIFTING ROCK OFF A CONVEYOR TO PLACE IT IN A BUCKET AND FELT A PULL IN HIS LEFT SIDE IN THE STOMACH AREA.

ROCK FELL OFF PRODUCTION TABLE ONTO THE BACK OF HIS LT. FOOT (HEEL AREA); IT BROKE A COUPLE BONES IN HIS FOOT.

Ee picked up rock and it twisted on him; pulled muscle in left arm.

Walking across grating, grating gave way & his leg went through.

EE PUT ROCK IN CHOPPER: ANOTHER EE PUSHED ROCK TOWARDS CHOPPER FROM OTHER SIDE: CAUGHT HIS FINGER BETWEEN ROCKS: NO BLEEDING, BUT COULD NOT MOVE-POSSIBLY BROKEN.

EMPLOYEE WAS MOVING ROCKS ALONG CONVEYOR AND WAS BEING CARELESS. HE SMASHED HIS LEFT RING FINGER BETWEEN TWO OF THEM; LEFT GASH IN HIS FINGER.

EE WAS MOVING ROCKS ALONG CONVEYOR; HE WASN'T PAYING ATTENTION AND SLAMMED ONE ROCK INTO ANOTHER (WITH HIS RIGHT PINKY FINGER IN BETWEEN); RESULTED IN DEEP CUT AND BROKEN BONE, LEADING TO AMPUTATION (PARTIAL).

EE STEPPED OFF LEDGE & SLIPPED, BECAUSE HIS BOOTS WERE WET. HE TRIED TO STEADY HIS FOOTING AND CATCH HIMSELF, BUT TWISTED (POSSIBLY BENT BACK) HIS FOOT. BIG TOE ON RIGHT FOOT WAS SWOLLEN & DISCOLORED.

Ee threw diesel on hot coals while standing in front of wood-burning stove; he was wearing parachute pants, which melted onto his skin when the fire shot out of the stove.

EE WAS MOVING ROCK OFF CONVEYER; ONE ROCK FELL AFTER PASSING THROUGH THE CHOPPER, HE TRIED TO STOP IT AND IT FELL ON HIS RIGHT PINKY FINGER.

EMPLOYEE WAS CHANGING OIL ON OUR KOMATSU; HE WAS WEARING SAFETY GLASSES. HYDRAULIC OIL SPILLED OUT ONTO HIS FACE AROUND THE GLASSES AND INTO HIS RIGHT EYE.

EMPLOYEE WAS MOVING ROCK ALONG THE CONVEYOR WITH OTHER EMPLOYEES; HE DID NOT GET HIS HAND OUT OF THE WAY FAST ENOUGHT (HIS WORDS). A ROCK SMASHED AGAINST HIS RIGHT HAND AND BLEW OUT THE FINGER NAIL ON HIS RING FINGER.

ROCK FELL ON HIS HAND AFTER GOING THROUGH THE CHOPPER, BLEW OUT THE FINGERNAIL ON HIS LEFT HAND (RING FINGER).

ROCK SLIPPED FROM CRANE AND LANDED ON HIS HAND.

EE WAS HELPING MOVE ROCK ALONG CONVEYOR; ROCK FELL ON HIS HAND AND CUT PINKY FINGER OF LEFT HAND.

EE WAS LIFTING ROCKS ONTO TABLE, HE DIDN'T LIFT ONE HIGH ENOUGH AND SMASHED HIS LT. HAND BETWEEN ROCK & TABLE. HE SMASHED THE FINGERNAIL ON HIS FIRST FINGER.

EMPLOYEE'S HAND GOT CAUGHT BETWEEN A ROCK AND THE ROLLERS ON THE CONVEYORS; CAUSED LACERATION IN RING FINGER OF LEFT HAND.

EE THINKS HE PULLED MUSCLES IN HIS RIGHT WRIST WHILE LIFTING, MOVING ROCK.

EE WAS STANDING ON SLAB STICKING OUT FROM A PALLET TO ADJUST STONE AT TOP OF PALLET. SLAB GAVE WAY/BROKE BENEATH HIM; CAUSED HIM TO FALL AND SCRAPE LEFT LEG ON BROKEN PIECE OF SLAB ON HIS WAY TO THE GROUND.

WHEN FLIPPING ROCKS MANUALLY (BECAUSE HOIST WAS DOWN), EMPLOYEE THINKS HE PULLED MUSCLE IN HIS BACK WHEN COMING DOWN WITH ONE OF THE ROCKS.

PICKED UP ROCK TO MOVE HOIST UNDER IT; DIDN'T THINK IT WAS THAT HEAVY; DROPPED IT ON HIS FINGER; BLEW FINGER NAIL OUT ON RIGHT INDEX FINGER.

ROCK IN HOIST SHIFTED AND FELL; HOIST SWUNG BACK AND HIT EE IN FACE; CAUSED LACERATION AT CORNER OF RIGHT EYE.

EE WAS MOVING ROCK ALONG CONVEYOR; HAND SLIPPED AND FINGER GOT CRUSHED BETWEEN ROCK AND ROLLERS.

EE WAS MOVING ROCK FROM TABLE TO BUCKET, HE PULLED THE MUSCLE IN HIS RIGHT SHOULDER AND UPPER BACK.

EE SET HAND ON TOP OF CHOPPER TO REST; OPERATOR WAS TALKING WITH ANOTHER EMPLOYEE AND PULLED LEVER TO CHOP ROCK; RIGHT HAND WAS SMASHED BETWEEN PARTS OF CHOPPER.

EE WAS UNLOADING ROCK FROM JOINT SAW AND DROPPEDONE ON HIS RIGHT FOOT AND BROKE IT

ROCK BROKE; EMPLOYEE TRIED TO CATCH IT AND MISSED; ROCK FELL ON HIS SHIN; THE ROCK BRUISED AND ABRASED HIS SHIN ON HIS LEFT LEG.

MOVING ROCKS ON ROLLER TABLE AND DIDN'T GET HIS FINGER OUT BETWEEN 2 ROCKS.

LIFTING VCUUM DEVICE THAT SUCKS DUST WHEN DRILLING ROCK. HE LIFTED TO MOVE TO ANOTHER ROCK, WHEN HE STRAINED HIS BACK.

A SMALL HEEL FELL OFF TABLE HITTING HIS CHIN, GIVING HIM A DEEP BRUISE.

HE WENT TO PICK UP ROCK CLASP WAS HOLDING IT IN WRONG PLACE AND PINCHED HIS RT INDEX FINGER.

2 EES WERE SETTING ROCK ON TABLE THEN LAYING IT FLAT AND GOT OTHER GUYS. EE DIDN'T GET HAND OUT OF THE WAY. BROKE TIP OF RIGHT MIDDLE FINGER AND MASHED RING FINGER BESIDE IT.

EE WAS PUSHING ON ROCK & END OF ROCK BROKE CAUSING EE TO SPRAIN WRIST.

PERSON WAS LIFTING ROCK WITH 2 TON CRANE, ROCK SLID THROUGH STRAP AND HIT EMPLOYEE ON RIGHT ANKLE.

EMPLOYEE WAS HAMMERING ON ROCK AND ROCK FLIPPED OFF TABLE ONTO HIS TOE.

USING AN ELECTRICAL HOIST AND ROCK TWISTED AROUND HITTING HIS HIGH SHIN AREA AND PART OF KNEE.

PUSH ROCK AND GUIDE IT. ON TABLE AND WRIST STARTEDHURTING FROM PUSHING ON ROCK.

HAMMERING ON ROCK AND HURT WRIST. WHILE HAMMERING ON ROCK.

WAS LIFTING ON A ROCK AND PULLED HIS BACK.

WAS PULLING ELECTRICAL WIRE THROUGH CONDUIT AND FELL OVER A 4 X 4 BOARD. HE HIT HIS HEAD ON THE UNLOADING TABLE. WORKED THE REST OF THE DAY. HE WENT TO THE ER ROOM ON 11/5/00 FOR HEADACHES, PU LLED MUSCLES IN RIGHT SHOULDER AND NECK.

WAS STANDING ON TOP OF TABLE TRYING TO MOVE A ROCK AND FOOT GOT CAUGHT BETWEEN THE ROLLERS. SPRAINED ANKLE.

FLIPPING A ROCK ON IT'S SIDE. HAND SLIPPED OFF AND HE TRIPPED OVER A PALLETT AND HIT HIS SHOULDER. HE LOST HIS BALANCE.

HE WAS CLEANING ALLEY WAY AND A ROCK FELL OFF OF THE TABLE AND HIT HIS LEFT KNEE ON HIS LEG.

HOLDING BRACES WHITE EE WAS WELDING. WELDING FLASH BURT EYES AT SIDE OF GLASSES.

STOPPED FORKLIFT TO CLEAN GLASSES AND THE WIND WAS BLOWING. THE WIND GUST BLEW INTO HIS EYES.

A ROCK FELL OFF OF THE TABLE AND HIT A 2 BY 4, THE 2 BY 4 FLEW UP AND HIT HIM IN HIS RIGHT EYE.

WAS PICKING UP A ROCK, WHEN THE ROCK SLIPPED OUT OF HIS HANDS AND HE TRIED TO JERK IT BACK UP, HE PULLED A MUSCLE IN HIS LEFT LOWER BACK.

UNLOADING A ROCK FROM LITTLE CHOPPER UNLOADING TABLE, AND ROCK FELL OVER ON HIS RIGHT MIDDLE FINGER.
