EE was coming out of the MCC on the 3rd floor of GR3 plant and looking up, when he tripped over a pump/motor that is welded to the floor. He fell on his right side. He was off 3/10,3/11, and 3/12 due to his injury. Returned to work with dr note 3/14.

Employee was carrying a 6" hose and slipped on another hose buried in snow. He fell and hurt his shoulder. He restricted to no overtime for 2 weeks on 6-18-08.

Employee fell while climbing out of the cab of an LHD injuring his right shoulder.

Employee was using a pry bar on a railroad car when his foot slipped and he fell backward onto packed snow. Employee had pain to lower back, right hip, and right side of head. (Restricted to no overtime until resolved on 1/24/08).

EE was reading clipboard, when he slipped and fell on the ice. EE struck his right hip on the ground and indicated that his clipboard struck his ribs (left front). EE limited to no overtime for 2 weeks.

Oiling gearbox for cooling tower fan in GR3, he fell from a ladder straining his right arm and shoulder. Restricted duty: no overtime.

Employee was walking backwards and tripped on a rock injuring his back.

EE lost footing and hit hand rail on left side.

EE was walking down stairs and twisted his ankle. EE restricted to no overtime on 9/14/07.

Employee was stepping off of the crossover on the FCT and hyperextended his lower back.

Employee stepped on a rock in a hole and fell down.

EE twisted his upper back while carrying belt roller.

Employee was tying a hand brake on a single rail car when he lost his grip and fell backwards. He landed on his back on the rail.

Employee was dragging a piece of tubing and tripped over a muck pile. He was originally diagnosed with a back strain. On 3/30, employee was re-diagnosed with a compression fracture which made this injury reportable. 3/30/07 is the date the injury became reportable.

Original Report: Employee was pulling on a hose while cleaning the showers and felt a pull in his hip.**Amended Report: Employee was pulling on a hose and fell down, injuring his shoulder. Employee continued to work his regular job until 3/22 when he went off for surgery.**

Employee was climbing onto cat and slipped on the step straining his left quadrisep muscle.

Employee was stepping out of mantrip and strained his lower back.

Employee was walking out the door and slipped on the ice and hurt his neck. He is restricted to no overtime.

Employee slipped and fell onto grating and lacerated his hand.

Employee alleged that while walking SB19 beltline, he stepped in hole twisting his left ankle.

Employee slipped on a stair and hit his elbow requiring sutures.

Employee was stepping from mantrip to top of loader center, slipped and fell on left wrist.

Employee was stepping off of jeep and lost his footing causing him to twist his ankle. Employee did not seek any medical attention until 10/17/06. Employee is on a no overtime restriction.

Employee stated that she got out of vehicle and slipped on snow & ice.

Employee was walking and stepped on uneven surface and twisted his ankle. His ankle continued to bother him so he went to see a doctor. Employee had previous problems with his ankle and the doctor felt it warranted surgery. Surgery scheduled for 6/20/06. No restricted duty or lost time to this point.

Employee was pulling on miner cable with a hot stick when hot stick slipped off of cable. He fell folding his right leg under his weight. He said he felt something pop and could not put weight on it.

Man was working on the 4th floor of a surface mill when he was asked to go to the bottom level and act as a hole watch. Although the stairs were right next to the work area, he went approx. 60' to a manlift belt. He caught his right heel on the upper floor grating as the platform went down, tripping him back and causing him to fall off the platform about 7' to the deck below.

EMPLOYEE WAS WALKING DOWN STAIRS AND HOOKED TOE OF HIS BOOT ON STAIR GRATING CAUSING HIM TO TWIST HIS KNEE. HE REPORTED THE INJURY ON 8/24

EE WAS WALKING AND SLIPPED ON WET FLOOR INJURING RIGHT ANKLE

EMPLOYEE ALLEGED THAT WHILE WORKING ON THE LIGHTS OF THE FRK30, HE STEPPED OFF OF FORKLIFT ONTO THE ELECTRIC SHOP FLOOR AND SPRAINED HIS LEFT KNEE. EMPLOYEE DID NOT MISS ANY TIME BUT HAD SURGERY ON 10/24/05.

EMPLOYEE ALLEGED THAT HE WAS TRYING TO PULL A PIN OUT OF A SPLICE ON THE MAIN ROAD OF PANEL SB10 WHEN HIS FOOT SLIPPED. THIS PULLED THE MUSCLE OR NERVE IN HIS THIGH. EE WORKED ALL HIS SHIFTS UP UNTIL 6/2/05. EE TOOK VACATION 6/3/05, 6/4/05 & 6/5/05.

Employee was walking on the outby offside of Flexible Conveyor Train in panel SB10 when he slipped and fell, catching himself with left hand on the running FCT-2 belt which pulled his hand into the roller on FCT-2.

EE was walking and fell. When ee attempted to catch himself, he injured his shoulders.

Employee caught his rt. boot under grating & twisted his rt. knee. Incident occurred on 3/6/05 & EE did not see a Dr. until 3/14/05.

EMPLOYEE WAS SERVICING M20-3 BELT DRIVE, HE STARTED TO WALK OUT OF THE AREA & TRIPPED ON SOMETHING. HE REACHED OUT TO BREAK HIS FALL & FELT A BURNING SENSATION IN HIS RT. SHOULDER. EE DID NOT MISS ANY WORK BUT HAD RESTRICTED DUTY.

Employee slipped on some ice and twisted his back.

EMPLOYEE WAS WALKING ALONG RAILROAD CARS TO SET BRAKE ON THE SCAMP AND TRIPPED ON UNEVEN SURFACE, TWISTING LEFT KNEE.

EMPLOYEE WAS WALKING AND SLIPPED ON FOAMY WATER, SPRAINING RIGHT ANKLE.

Employee was walking across surface shop to adjust welder and tripped over welding leads.

EMPLOYEE STATED HE WAS HANGING VENTILATION TUBING WHEN HIS FOOT SLIPPED OFF A ROCK.

EE STATED HE WAS WALKING WHILE CARRYING A PIECE OF METAL PIPE AND TRIPPED OVER A ROCK.

EE stated he stepped in some mud when he tried to pull his boot out he twisted his lower back.

EE STATED HE WAS CLIMBING LADDERS ALL DAY AND EITHER PULLED OR STRAINED HIS KNEE. EE CONTINUED TO WORK - FOUND OUT EE HAD SURGERY ON 6/3/04.

EE STATED HE WAS WALKING FROM CHANGE ROOM TO CRUSHER BUILDING TO GO TO WORK WHEN HE SLIPPED ON ICE IN ROAD AND FELL.

EMPLOYEE STATED HE WAS STEPPING OVER A CONDENSATE PIPE WHEN HE FELT A POPPING SOUND IN HIS KNEE. EMPLOYEE WORKED UNTIL 3-1-04 WHEN HE WENT INTO THE HOSPITAL FOR SURGERY.

EE WAS STANDING ON LADDER, WHILE ATTEMPTING TO COVER A LEAKING PIPE-EXTENSION LADDER SLID OUT FROM UNDER HIM CAUSING EE TO FALL.

EE WAS DOING HIS NORMAL JOB CUTTING HALF PASS IN PANEL, WAS WALKING OVER HALF PASS AND TWISTEDHIS LEFT ANKLE.

EE WAS GATHERING BOTTOM ROLLERS IN PANEL SB05 AND LOADING THEM INTO MANTRIP. EE TURNED AND HYPEREXTENDED HIS RIGHT KNEE CAUSING HIM TO FALL TO THE GROUND.

EE WAS HELPING BOLTER OPERATOR HANG TUBING IN PANEL A98. AFTER HANGING TUBING, HE WALKED BEHIND BOLTER, TRIPPED OVER A ROCK OR THE BOLTER CABLE, INJURING HIS RIGHT KNEE.

Employee was walking and tripped over rock causing injury to left knee in the mine. This accident occurred in in 2003. The injury was first reported to MSHA in 3-15-2005. The employee returned to work from injury 7-11-06. Employee is currently works on the surface as a dozer operator. Employee is going into surgery on left knee 9/25/2019.
