Showing all 3Employee's foot slipped on catwalk at entrance to surge pile tunnel. Fell on catwalk guardrail with right shoulder. Pain in right shoulder. Employee received some physical therapy treatments but missed no work days. Injury was later determined to be torn muscle. Surgery to correct performed on 08/17/15.
Employee was shoveling material from under a conveyor. Due to wet and cold ground conditions employee's feet slipped on ground and hit his shin on the truss of the conveyor. Injury resulted in laceration on the right shin that needed three stitches.
