Climbing out of the Mechanics Service Truck. Stepped on Ladder, ladder feel to one side, causing employee to fall. Landed between rungs of ladder; then folded up on employees left leg causing 1 bone to break and another bone to fracture.

Showing all 8Employee was climbing out of truck. She grabbed the handle, but slipped off the foot hold and fell. She struck her lower back on the top step of the ladder. She felt fine at first, but later experienced numbness & tingling in her legs. She requested & received emergency transport to the hospital. She is currently not working as we have no "restricted duties" for her to perform.
EMPLOYEE SLIPPED AND TWISTED HIS BACK WHILE CLIMBING DOWN FROM A ROCK TRUCK.
Employee was helping change iron in the jaw when he fell and pulled a muscle in his arm.
EE WAS ON THE POWER CHUTES, CHANGING THE SCREENS. HE SLIPPED AND FELL TO THE GROUND (APPROX. 7.5'), BREAKING HIS LEFT WRIST.
EE WAS CLIMBING INTO THE LOADER. HIS HAND SLIPPED OFF THE HANDRAIL, AND HE FELL BACK. THE FOLLOWING MONDAY, THE EE HAD SO MUCH BACK PAIN THAT HE WAS UNABLE TO WORK AND NEEDED MEDICAL ATTENTION.
EE WAS WALKING AND STEPPED INTO A SNOW-COVERED HOLE, TWISTING AND STRAINING HIS LEFT ANKLE.
EE WAS STEPPING OUT OF TRUCK. HER BOOTS WERE MUDDY AND SHE SLIPPED ON THE TRUCK STEP. SHE FELL AND HER RIGHT ANKLE WAS SPRAINED AS IT LANDED ON A ROCK.
