Fall onto or against objects

Transporting Rhino Lining, when a piece of lining fell off. Got down to reload, when he stepped back and tripped; landing on fork hangers, arm hung up and injured right shoulder.

Caught in, under or between a moving and a stationary object

Removing drive coupling on electric motor, when coupling slipped, trapping middle finger on right hand in between couplings; smashing finger

Fall from machine

Climbing out of the Mechanics Service Truck. Stepped on Ladder, ladder feel to one side, causing employee to fall. Landed between rungs of ladder; then folded up on employees left leg causing 1 bone to break and another bone to fracture.

Replacing backup light on rock truck. Fixing cover with 3/4"" screen. Had to bend the screen 45 degrees, screen popped and swiped face, nose and upper lip.

The miner was digging out around the cone discharge when he felt his back pop out. He went to the emergency room, and then to a clinic for care. He currently released to restricted duty, but is not working.

Employee was climbing out of truck. She grabbed the handle, but slipped off the foot hold and fell. She struck her lower back on the top step of the ladder. She felt fine at first, but later experienced numbness & tingling in her legs. She requested & received emergency transport to the hospital. She is currently not working as we have no ""restricted duties"" for her to perform.

Employee was helping to unload several large timbers from a tractor/trailer and strained his back. The injury was such to require 4 to 5 chiropractic treatments.

EE WAS CLIMBING INTO THE LOADER. HIS HAND SLIPPED OFF THE HANDRAIL, AND HE FELL BACK. THE FOLLOWING MONDAY, THE EE HAD SO MUCH BACK PAIN THAT HE WAS UNABLE TO WORK AND NEEDED MEDICAL ATTENTION.

Crew was loading a used oil barrel, using a loader & chain to lift the barrel. barrel swang thru the air, came toward the ee, caught him off gaurd, causing the ee to reach up and trying to stop the barrel from swinging. the weight and force of the barrel hitting the ee's outstreched arm caused a strain in his back, for which he will need chiropractic care for several weeks.

EMPLOYEE WAS PREPARING SOME FIBERGLASS WITH A HAND GRINDER WHEN HE FELT A FOREIGN OBJECT ENTER HIS RIGHT EYE.

EE WAS WALKING AND STEPPED INTO A SNOW-COVERED HOLE, TWISTING AND STRAINING HIS LEFT ANKLE.

WHILE INSTALLING A 4"" ELECTRICAL WATER PUMP AT THE SAND PLANT, THE EE ATTEMPTED TO PLACE THE SUCTION HOSE BY HITTING IT WITH THE HEEL OF HIS RIGHT HAND. AFTER 10 DAYS OF OBSERVATION, THE EE DE CIDED TO SEEK MEDICAL TREATMENT FOR HIS RIGHT HAND, & FILE A WORKER'S COMPENSATION STATEMENT WITH OUR OFFICE.

EE WAS TAPPING (DRILLING) OUT A HOLE ON THE COUNTERWEIGHT, WHEN THE DRILL BIT STUCK & THE TORQUE PULLED THE EE'S RIGHT SHOULDER.

EE WAS REMOVING COOLING FAN WITH HAMMER AND PUNCHED COOLING FAN. HE MOVED AND STRUCK HIS HAND WTH THE HAMMER.

EE WAS WORKING TO CLEAR THE JAW CRUSHER OF A LARGE BOULDER THAT JAMMED THE CRUSHER AND PULLED A MUSCLE IN HIS LOWER ABDOMEN. EE DID NOT REALIZE THE EXTENT OF HIS INJURY AT THE TIME AND DID NOT REPORT IT TO HIS SUPERVISOR.

MINER WAS SHOVELING OUT TAIL PULLEY AREA, ROCK BOUNCED FROM 1036 PIONEER JAW, STRIKING MINER ON TOP OF THE LEFT SHOULDER, CAUSING SMALL BRUISE AND CONTUSION.
