Employee was walking slope on a leach pad and plumbing solution pipes. While walking on the slope the material was wet and caused the employee to slide and then tumble down the slope. The employee had a strained neck and right ankle due to the incident. The Doctor recommended that the employee take three days off of work.
The employee (EE) was using a paint stick to begin marking boot holes following a heading inspection. The EE's right foot slipped on uneven ground causing their weight to shift left. The left foot also slipped, and the EE began to fall to the ground. A rock came between the ground and leg just above the left calf area. The impact resulted in a fracture to the lower left fibula.
While dismounting the grader, the operator lost grip and fell.
While working in a squatted position on an engine at ground level, Mechanic lost balance landing on a pipe.

X-Rays revealed that the employee had a fractured coccyx. the employee was released to full duty and will return to work on next scheduled shift, June 15th.
Employee was dismounting a track dozer and tripped forward and felt the knee pop and was taken to Hospital in Ely, NV. Xray performed with negative findings and released to full duty with. Followed up with Ortho and MRI was completed on 04/23/2024. MRI results read on 04/24/2024 with the ACL and meniscus was damaged. Released to restricted duty on.
Employee was walking back to work area after lunch. While crossing the roadway they slipped on the ice and broke their tibia.

Case began as a first-aid only case on 05/25 and was modified to reportable injury on 07/07. Laborer using a vice grip clamp attached to the liner to help guide the liner. The employee tugged on the liner and the clamp came loose. The employee lost their footing and fell backward on the slope. The employee braced themselves with their right hand and received a strain to the wrist.
Employee fell during a line out meeting and broke a front tooth.
