Employee stepped on a rock that was next to the rib. When rock shifted, employee lost his balance and hit his left shoulder on rib. Employee had bruising and redness to left shoulder and returned to work. On 11/8/12 employee received medical treatment for shoulder pain and received work restrictions.

MINER OPERATOR HAD GOTTEN OFF THE MINER TO ADJUST THE VENT TUBE. AS HE RETURNED TO THE MOINER, HE SLIPPED, HE REACHED TO CATCH HIMSELF GRABBING THE BOLTER SUPPORT AND HIS FINGERS WAS CAUGHT BE TWEEN THE JACKS, PINCHING HIS FINGERS.
EE WAS WELDING ON A BRACE 3 FT. OFF THE GROUND. WHEN HE STEPPED OFF THE PLATFORM, HE LOST HIS BALANCE AND FELL. EE REACHED TO CATCH HIMSELF FROMFALLING CAUSING HIS ARM TO DISLOCATE.
