It is believed that the customer truck driver fell from EE's trailer to the ground while stepping from the trailer to the safe rack. The fall resulted in multiple injuries requiring medical attention and a loss of work days.

Winter blizzard and changing weather conditions created slippery surfaces. Ice developed on top platform dust collector and created a slippery surface. After performing maintenance task to thaw frozen air lines, employee sustained a left ankle injury after ascending ladder and stepping on the top platform for accessing the dust collector resulting in restricted duty.
Employee fell while preparing to fuel a D6 dozer after a ladder/stool failed, resulting in a right ankle fracture.

While performing cleanup, the floor plating the employee was standing on fell into the water ditch resulting in a fractured leg.
While staging material for a maintenance task, the employee tripped over a chain hoist and landed awkwardly on a guard the employee had just removed. The resulting injury was a fracture to the fingertips of both the left ring and middle fingers, as well as five sutures.

The diagnosis is still outstanding due to lack of specialist care availability and a request to reclassify this injury may be made in the near future.

While attempting to clean the windshield of a haul truck in contradiction of company policy the employee fell to the ground. The fall resulted in a fractured rib and facial bones.
The employee was working in the mill room and when they turned they tripped over a cord falling on left side causing discomfort to hip left hip and thigh.

The injured employee was stepping down off of a step stool when employee began to turn. Employee's foot slipped on wet slick concrete and employee fell to the ground. The injured employee's right arm impacted the concrete ground hyperextending elbow and fracturing radial (arm) bone.
Employee slipped and fell while holding the end of a 60-80 lb pump. EE's left hand was caught in between the pump and the concrete ground injuring EE's left middle finger.

Contractor employee was climbing the ladder steps on a CAT 50 ton haul truck to check the oil. A foot slipped and they hit a knee. Employee informed a supervisor the next day and was taken to the Dr. The Dr. said the knee was bruised and employee was placed on light duty.
Miner fell walking next to haul truck, and landed on right elbow and experienced right shoulder discomfort.
As employee exited a rubber tire yard forklift, foot missed or was partially on the step and they lost balance and fell backwards. Hard hat fell off and they struck the back of head on a vertical structural beam resulting in a 3" long laceration. Employee was treated at the hospital with 3 staples to close the wound and a tetanus shot.
Employee was walking down a set of internal stairs when left foot slipped. The employee was holding both handrails. The employee's weight settled on left arm and shoulder and the employee felt a "popping" sensation in left shoulder. As the employee caught self and straightened up, EE again felt shoulder "pop" back into place.

Miner experienced right knee discomfort as EE planted EE's right foot on the ground when EE was dismounting a John Deere 460e haul truck.
Cable for lower flexible step on Volvo A35D Water Truck broke when employee stepped up on it. Employee did not fall to the ground, but strained EE's lower back by holding on and lowering self.
EE claims injury occurred on 8/5/19. EE reported to Supervisor until 8/25/19. EE was taken for medical eval on 8/26/19, released to full duty. On 9/10/19 EE complained pain returning and was transported for second medical eval and was released to full duty. MRI was scheduled & was obtained on 10/10/19. 11/5/19 EE returned for further medical evaluation, assigned restrictions.
Employee tripped on the gravel path step between the truck shop and the office. Video shows that EE didn't lift foot high enough for the 3rd step. The tip of EE's boot hit the step causing EE to trip and fall forward hurting left shoulder. The same shoulder that EE was currently going through physical therapy for, for a non-work related injury.
Employee fell to the ground after tripping over a water line near Conv. #1 TP while washing down with a water hose. Employee put out right hand to break the fall and reported shoulder pain. Doctor diagnosed a non displaced fracture on July 23rd.

The Operator was performing maintenance on a gear box that was located on the ground at the Wet Plant Department. Operator was bent down while performing the job, when Operator raised up and stepped back, lost footing and slipped. As Operator was slipping, forehead encountered the gear box motor mount, resulting in a laceration.
While exiting a front end loader the employee's boot tread got caught on the bottom step of the ladder causing employee to twist awkwardly resulting in a sharp pain in left knee. Employee went to a doctor for an evaluation on 10/16/2018 and was put on restricted duty.
While attempting to connect an SRL to the harness, the employee lost balance and fell (same level) striking left knee on a toe board. The resulting injury was a laceration requiring 5 stitches.
Employee climbed down from haul truck, jumped off bottom step, landed on uneven ground with right foot and rolled ankle. Employee was put on crutches/restrictions on 08/21/18.

EE was cutting screen when EE stepped back to evaluate work and their right heel got caught in screen causing them to fall and strike the bucket on the skidsteer bucket that was parked in the vicinity.
Employee was attempting to dismount a 15KVA electrical transformer approximately one foot off the ground near the scale station. Employee lost balance which resulted in the employee hitting head on a piece of unistrut used to mount transformer. Employee WAS wearing hard hat and safety glasses.
On 01/31/18, EE reported that while working in the rail yard ee slipped on ice & twisted left ankle. EE was not wearing ice cleats at time of the event. EE was offered & refused medical treatment. On 02/05/18, a medical evaluation and diagnostics revealed a fracture on left ankle. EE was put on restricted duty as of 02/05/18.
Slip and fall injury to right knee occurred. Employee did not initially want any medical treatment. Later on employee had knee swelling and drained by Dr on 12/6/2017. Doctors diagnosis was possible meniscal tear on right knee now made this a reportable.
Employee stepped onto stairs platform while exiting the office trailer. Platform shifted unexpectedly. Employee experienced a pain in the left hip.
