The employee was walking on an icy surface and EE's feet went out from under EE causing EE to land on EE's bottom. The employee dropped EE's right arm to soften the fall and injured EE's shoulder. The employee has seen several doctors since Feb. 16 and now will have surgery.

Miner was hanging utilities and slipped on wet ground, falling and landing on a jagged rock with his right forearm, causing a cut near the elbow, requiring 4 stitches.
Driller slipped on a small bit of grease and fell straining his knee.
The injured was dismounting the rock truck when his right knee gave out. He fell to the ground, twisting his knee. He was sent to dr for assessment and nothing was foung. He was later sent for further assessment to an Orthopedic specialist due to knee pain. He was given a knee brace on 5/13/10 and RTW on 5/17/10.
Injured & his partner were on a 6 x 6 raft and pulling themselves toward the dewater pump to do the monthly P.M. The partner pulled on the rope used to move them forward when the raft tipped sideways. The injured worker grabbed the wall to support himself and in doing so injured his right hand. The water is 12" deep.
The employee was doing seasonal maintenance on the swamp coolers where the mine employees live. He was leaving the roof to go down the ladder. When the employee put his weight on the ladder, the safety catches released & the employee fell (maximum fall 11'2" from eave of trailer to the ground). Injuries were four fractured ribs and a cut spleen.
While walking, the mechanic stepped into a hole and fell to the ground striking a rock with his knee. The extent of the injury diagnoses was received on 8/5/08 and that resulted in a reportable inujry.

EMPLOYEE SLIPPED ON A WET FLOOR AND HIT HIS HAND ON A LOCKER IN THE CHANGE ROOM.
LOST HIS BALANCE WHILE MANUALLY POSITIONING A DRUM FOR A BARREL GRAB AND FELL FROM A FLATBED TRAILER.
THE EE LEFT THE BUILDING TO DO A YARD TOR, NOTICED THAT THE SNOW HAD BEEN REMOVED BUT DID NOT NOTICE THE FILM OF ICE. HE SLIPPED AND FELL. THAT DAY, DR TOLD HIM HE HAD A CONTUSION, LATER IT WA S DETERMINED HIS INJURY IS MORE SERIOUS.
EE HAD BEEN CLEANING MUD OUT OF AN INTERMODAL CONTAINER USED TO SHIP PROCESS FEED FOR THE MILLWHEN EXITING THE CONTAINER, HE KNELT DOWN PLACEDHIS HAND AT THE EDGE TO HOP OUT. WHEN HE TRANSFE RRED HIS WEIGHT TO HIS HAND IT SLIPPED BECAUSE OF MUD, AND HE FELL APPROX 5' LANDING ON HIS FOREHEAD
