The employee was going to the pumps in the quarry to check their status. EE stepped over a pipe and observed a snake in the shadow of the pipe. As EE turned to get away from the snake, EE caught boot on the pipe and hit knee and hand on the ground.

The employee was walking to a loader to begin operation. The ground was still wet from rain where the loader was parked. As the employee approached the loader (approximately 3' from the loader) EE slipped. EE was diagnosed with a contusion and an abrasion on EE's right arm.

While walking out of the administration building; descending the north stairs the employee felt a sudden sharp pain in his right knee. He did not know the cause of the pain; there was no loss of balance or impact. The employee is on light duty as a result of the injury.

The employee was cleaning out an airslide when the cement free flowed out and the employee tried to climb down a ladder. Due to the free flowing cement there was no visibility. The employee fell off the ladder approximately 7-8 feet up to a catwalk below. The employee sustained an abrasion to the right bicep.

Employee was cleaning the feed end of two finish mill using a shovel and a wheel barrow. The employee entered the east door pushing the empty wheel barrow and set it down. The employee began walking on the clear walkway to retrieve a shovel when his left foot got caught on the grating causing him to trip. The employee sustained a sprain and minor contusion to the left knee.

EE was working on and elevated platform removing duct work. When the duct work was removed, a small hole approximately 2'X 2' remained. The employee stepped into the hole, causing him to fall against the adjacent hand rail. The accident resulted in two fractured ribs.

Employee was walking down steps on west side of cooler, he tripped on step and felt pain in his right knee, diagnosed with knee strain. Employee was able to fully perform job functions and returned to full duty. Pain continued in knee, sent for MRI. Employee was sent for surgery December 30, 2010, arthritis aggravated by trip on the step.

EE was using a jackhammer to clean inside a hopper when he decided to sit and take a break to wipe sweat from his head. He removed his hard hat and gloves and wiped sweat off. When he stood up, he slipped. When he slipped, he grabbed onto a piece of metal in the hopper to prevent the fall and the metal cut his left hand. The laceration required 5 stitches to close.

Employee was walking on roadway, stepped off roadway and twisted right knee. Employee was sent for evaluation, he was able to fully perform his job until 08/24/2010 when pain increased in knee and he was sent for MRI. Diagnosis was torn meniscus with no work restrictions placed on employee. This injury was deemed questionable and has now been deemed work related and reportable.

Employee was on the second step of a step platform 16 inches off the ground, he turned to lift down a section of 3 inch pipe that had been cut away from the main line. As he turned his right foot stepped on his left foot causing him to slip off the step, his left foot caught under the step twisting his left leg. He felt pain in his left shin, transported to hospital.

Employee was walking through Packhouse Warehouse to inspect a sump pump, he stepped in a cracked portion of concrete floor and twisted his right ankle. The broken concrete was 10 inches by 12 inches and 1 inch deep. Employee sustained sprained right ankle.

Employee was walking through area at new kiln, when he twisted his left ankle and foot. Medical evaluation revealed a fractured bone in foot. A cast was applied and work restrictions mandated. The employee is a manager and is able to perform his job duties within the restrictions.

While climbing into cab of dozer, employee slipped and grabbed ladder spraining his wrist. Subsequent evaluation 12/6/07 indicated that employee had sustained a torn ligament in his left wrist. Was on lite duty from this time. Surgury was on 1/7/08 at which point this becomes a lost time.

Employee tripped on welding lead and twisted both knees, diagnosis-strain. Employee returned to work full duty, 11/26/2007 MRI showed meniscus tears in both knees. On 12/14/2007 employee had surgery on left knee. Injury had been contested as a pre-existing injury non work related. it became work related as of this date.

Employee stepped down off platform at Kiln 2 Vent Fan, twisted knee and felt pain. He reported incident but refused medical treatment on this date. Follow up showed torn left medial meniscas in left knee. Surgery on 11/02/2006 made this a reportable lost time accident as of this date.

Employee twisted knee on 3/10/04, he did not think he needed medical attention. He went to the doctor on 5/26/04 and was returned to full duty same day. Knee continued to hurt employee, went back to doctor on 6/1/04 and on 6/4/04 doctor recommended PT to begin on 6/7/04. No loss time or restricted duty has resulted from this accident.
