Employee was checking manually the level in the silo on an evening when it had rained. Although he wore gloves his hand slipped as he leaned on the side of the silo. He lost his balance, hit the platform guarding and fell down the ladder he was on.
EMPLOYEE WAS STANDING ON A SMALL STOOL 12"HIGH WHEN SHE SLIPPED OFF AND FELL ON THE FLOOR. SHE SAID SHE WAS FINE & WENT BACK TO WORK. WE CHECKED ON HER 4-5 TIMES DURING THAT DAY, SHE DID NOT HURT UNTIL THE NEXT DAY. WE TOOK HER TO A DR. FOR AN EXAM & X-RAY.

EE TRIPPED OVER A HOSE AND FELL ON HIS RIGHT FOOT. HE SAW THE HOSE ON THE WALKWAY BUT FORGOT TO MOVE IT OUT OF THE WAY.
