Employee was walking up stairs; toe of boot caught on grate and caused employee to fall, hitting left arm/elbow on toe board. Employee declined medical attention at time of incident, but sent for medical attention at 2:30pm on 10/28/2015; advised to wear sling on left arm and not use arm for 2-3 days. Employee was not scheduled to work again until 11/02/2015.

Showing all 6Employee was patching a hole in a water pipe. He stepped across/stood over pipe to weld patch in pipe; right foot slipped and twisted right ankle.
Employee was checking oil and greasing WA600 loader. Stepped down and missed the step and fell to the ground, hurting lower left side of back, left hip and left leg. Employee thought he would be okay, so he did not go to the doctor at the time of the incident. On 11/27/2012, employee was still in pain and was sent to the doctor for evaluation.
Stepped over dirt hill and knee popped. Grabbed handrail to keep from falling. Went to emergency room. Only drawled one week workers comp.
Employee was walking across catwalk to his workstation at the water gun. He slipped and fell into the guardrail and mashed his hand on the guardrail with his body weight. He jammed his left index and middle finger.
THE LOADING SYSTEM KICKED OFF AT SEPARATOR PLANT AND THE EE WENT TO TRIP SWITCH WHEN HE TRIPPED OR SLIPPED ON STEP. HE FELL AND CAUGHT HIMSELF WITH LEFT ARM IN AN AWKWARD POSITION, DISLOCATING HIS SHOULDER.
