Employee was walking towards EE's vehicle because the mine shut down due to snowy weather conditions. EE slipped and fell on the snow in the parking lot near EE's vehicle. Physical therapy recommended to start on Feb 15.
Employee stated that as EE was climbing onto EE's loader EE's foot slipped off the ladder rung and EE felt a pull in EE's groin.

Employee walked to scale printer to clear a paper jam and slipped on the snow. The area around the scale house had been salted however it was still snowing and the path they took to the scale printed has snow accumulated on it.
The employee pick up a tool turned around to take a step, felt knee pop and fell to the ground. (The employee is now receiving Physical Therapy.)
The employee stated was climbing into the truck, EE's foot slipped off the step and struck left shin. Employee went to the doctor on 4-6-16 and was given restricted duty.

EE WAS HANGING A THREE STRAND #2 SIZE WIRE FROM THE 88' HIGH RANGER THE TOOL USED TO PULL THE WIRE TIGHT SLIPPED CAUSING THE WIRE TO PIN THE EE AGAINST THE CAGE CAUSING HIS SIDE AND RIBS TO BE BRUISED EE REFUSED TO SEE THE DR UNTIL 8-5-03.
