Accident type, without injuries

A fire consumed the the mine office trailer. Cause unknown. No one was present at the time of the fire. Fire department extinguished fire and notified owner of the property.

Fall from machine

Employee states they ""Went to the wrong side of the handrail thinking the ladder was on that side and it wasn't but it was too late"". Employee fell off of the ANFO rig suffering a right ankle sprain.

Struck by flying object

Injured states ""I forgot my knife so blaster was throwing their closed knife at pillar next to me which hit and opened on impact then bounce (sic) and stabbed (sic) me in the arm."" 3 stitches required.

Struck against stationary object

Injured was using a wrench to tighten a bolt on a crusher. The wrench slipped off and injured's right palm struck the bolt head. The resulting laceration required 6 stitches.

Employee stated that as EE was climbing onto EE's loader EE's foot slipped off the ladder rung and EE felt a pull in EE's groin.
