Employee was dismounting the Stationary ladder on an open top railroad car, he lost his grip or footing and fell approx. six feet. He landed on his left wrist and left hip, this fall fractured his left wrist.
Snow covered plastic tarp and was unable to see end and walked on snow covered tarp and slipped. Employee has had previous hip injury outside work.
He was stepping up into loader when the bottom cable step broke, caused him to hit his right side ribs on the ladder. He was ask to go to the doctor and he felt he was alright. Then on 1/4/2005 he felt a need to go. The doctor said he had bruised only.Then contacted him later saying he had fractured a couple of ribs. No lost time, he put a wrap around his ribs for a few days.
EMPLOYEE WAS COMPLETING A NORMAL TASK OF FUELING HIS EQUIPMENT WHEN HE LOST STABILITY OR BALANCE CAUSING HIM TO FALL FROM THE BOTTOM RUNG/STEP LEADING TO THE CAB OF THE LOADER. EE FELL APPROX. 2' ONTO HIS BACK. HIS RT. HEEL IMPACTED THE GROUND CAUSING A FRACTURED HEEL.

DRIVER SLIPPED OFF FLATBED WHILE GETTING DOWN FROM TRAILER AFTER TARPING LOAD. LOAD WAS PARTIALLY LOADED AT ONE AREA AND HE MOVED TO SECOND AREA TO GET REMAINDER OF LOAD. WHILE WAITING FOR SEC OND PART OF LOAD, HE DECIDED TO TARP LOAD.
EE WAS DESCENDING A VERTICAL LADDER. WHEN HE STEPPED DOWN TO THE PLATFORM (MAY NOT HAVE BEEN ON BOTTOM STEP), HE LOST HIS BALANCE AND FELL AGAINST HANDRAIL CAUSING INJURY.

THE EMPLOYEE HAD JUST COME BA CK TO WORK AFTER HAVING KNEE SURGERY ON THE SAME KNEE (PERSONAL INJURY). THE EXTENT OF INJURY IS UNCERTAIN, BUT EMPLOYEE MAY HAVE TORN LIGAMENTS.
EMPLOYEE WAS EXITING VOLVO L-330C LOADER AND SLIPPED ON SNOW AND FELL TO GROUND FROM STANDING POSITION CAUSED A CONTUSION TO HIS RIGHT HIP, NO LOST TIME BUT HAS RESTRICTIONS.
EMPLOYEE WAS COMING DOWN THE STAIRS IN A HURRY & LOST HIS FOOTING, FALLING ON THE STEPS. HE STRUCK HIS RIGHT SHOULDER, RIGHT SIDE OF NECK & UPPER RIGHT ARM WHEN HE FELL.
