Employee was retrieving tools from the RM500 milling machine and, as they were descending the ladder, their pant leg got snagged on the ladder step and EE lost EE's balance, falling approx. 5 feet to the ground. They landed on their right wrist, resulting in a wrist injury.
Employee was prying on a part. The part gave way, causing the employee to step backwards into a hole, sustaining a knee sprain.

On 5/19/2025 at 3:00 PM, after working on FM1, an employee was leaving the Mill area when attempting to step down onto a platform 2 feet lower than the Mill, and they sprained their ankle.
While exiting vehicle in parking lot before shift start, employee slipped and fell. Employee hit their head and had minor abrasion on right pinky finger. On 2/10/25, employee was diagnosed with concussion.
Employee slipped in the ice and caught self before falling. Strained the right shoulder. Physician prescribed physical therapy on 2/12/25 making it reportable.
While retrieving the blend silo #2 sample, there was a strong draft that caused the tech who was holding a 4ft sampling stick to be pulled against the air slide. Diagnosed with shoulder strain and provided prescription meds/light duty.
Employee tripped over a wheel chock rope and shattered their knee cap.
A Norrell Construction elevator technician fell while climbing preheater tower elevator car ladder. In an attempt to not fall, the technician grabbed the ledge above the ladder, resulting in lacerations to several fingers and 2 fractured finger bones.
Employee was welding bars inside the trommel, using 2x6 boards and 3/4" plywood to sit on while welding. As the employee attempted to stand up, one of the 2x6 boards broke causing imbalance and resulting in a strained hamstring, and was giving an anti-inflammatory prescription. Employee was returned to full duty.
EE suffered a bicep strain while descending down a fixed ladder
While accessing the loader, the employee lost their grip on the door handle and strained their back.
Employee was walking when they sprained their ankle.
Employee was walking and tripped, fell onto their left side and injured their left shoulder.
The employee was walking in the plant to perform tank level reading when employee slipped on the uneven muddy ground (caused by weather conditions) lost footing and twisted an ankle. Medical advice was given for cold therapy and elevation. The next day employee felt some pain and was referred for more exams when employee was diagnosed with a Left fibula fracture.
Bulk truck cement driver was descending stairs on safe rack when they tripped and fell from the 4th or 5th step from the bottom of the stairs to the ground.
Employee was walking across the rock floor in the dome and EE's knee twisted while walking. Prescription was issued 1/08/24 resulting in this becoming reportable.

Quarry employee reports was putting rollers on, bent over, slipped, and twisted their back causing pain to their lower back. Injury was considered a first aid until 2/20/2023 when employee was given a Prescription for 10mg cyclobenzaprine and 20mg prednisone.
Heavy Equipment Operator was walking around equipment performing pre-operation inspection while EE slipped on an ice patch, due to winter storm. The miner landed on EE's right side, flaring up an old shoulder condition. The injury was first aid until 02/14 when physical therapy was prescribed.
While working in the pug factory, the system became clogged The employee tried to disconnect using the pressure washer when the accumulated dust returned to EE. EE lost balance and fell, causing an ankle injury.
Fatality occurred at Ada plant site from a fall from raw silos. Cause is undetermined at this time.
EE reports EE was trying to pull the guard off of a counter weight due to material build up. Once the weight came loose EE reports EE fell backward and stuck out EE's left hand to brace self. EE is right hand dominant and has no open wounds from EE's fall. Resulted in closed fracture.
Access ladder slipped, causing employee to fall approximately 4 feet. The ladder was improperly secured.
3rd party owner operator (3PO) failed to appropriately raise the safety platform. 3PO failed to depress the safety pedal to disengage the lock while pulling on the raise rope. Rope popped, causing 3PO to fall onto the platform. 3PO complained of right knee pain. 3PO desired evaluation and transport via EMS.
Employee was tightening conveyor belt. As they were climbing down off of the work area the employee's hammer loop on employee's pants caught one of the bolts, knocking them off balance, and causing them to land on their right leg funny resulting in a non-displaced fracture. Reportable on 2/1/22.
Employee was walking along side of building and tripped over boot cleaning grate located near doorway. Employee fell and felt pain in their shoulder. Employee has began receiving physical therapy treatments, changing the classification from first aid.
EE finished unloading a tanker truck, the unloading hose was unhooked from the truck. The employee then removed the adapter and moved the hose prior to capping the hose. There was a loss of vacuum on the hose and a small amount of material spilled on the EE and ground. As the employee was maneuvering around the spill the employee slipped and landed on their right knee.
Employee was walking around in the scraper compartment of the mill and slipped in the reject chute. As EE slid down the chute EE's left arm (shoulder) overextended quickly. Reportable on 12/1/2021.
Crew was lowering material down to the employee with a bucket and a rope. The handle of the bucket broke and fell, bouncing off of the wall and striking the employee. When the bucket struck EE, it knocked EE off of a platform causing EE to fall on a pole.
Employee was exiting a skid loader and used the top edge of the bucket for foot placement while climbing down. Right foot slipped and caused fall forward resulting in the area just above the knee to come into contact with the edge of the skid loader bucket and foot slipped backwards off the bucket causing overextension of the knee. Recordable 11/26/2021.
EE tripped over step in walkway causing laceration to left knee. Laceration required stitches to close.
Employee states that while adding dry ice left foot slipped into a hole causing employee to do the splits. Employee complains of pain in the groin area and right lower abdomen.
The employee was working on green tagged scaffold approximately 8-10 off of the ground around a raw mill roll pair removing the hub off of the shaft. The scaffold boards were against the hub until the housing was removed allowing for a space at which time the employee fell.
An employee was climbing a short set of stairs into the office building when EE missed a step causing EE to fall forward. EE caught self on left hand causing a wrist sprain.
Employee was walking in the parking lot to a vehicle and stepped in a pothole. EE then stumbled and tried to catch self with EE's left arm. EE hit EE's head and neck on the concrete.
The Injured Employee lost their balance due to stepping on a rock. While falling, they turned and put their hand down to brace the fall, scraping the elbow and hurting the wrist/thumb on their left hand. This injury was being managed through various visits to Dr/Clinic but became Reportable on 8/5/2021.
EE was assisting with the removal of the burner pipe from the lance. Employee slipped and fell while balancing the pipe.
Employee assigned to housekeeping activities where employee was carrying wooden pallets. Part of the pallet fell from the employee's grip causing a trip and fall landing on both hands. Employee diagnosed with epicondylitis.
Production Utility Operator (PUO) was walking through the plant headed to the kiln area. Employee was near the equipment storage building when they slipped on snow and fell on left side. This incident was upgraded to a modified work duty on 02/19/2021.
