Showing all 8Employee was using water hose to clean out under crusher when there was a change in water pressure causing employee to fall forward. Employee received 5 stitches to LT eyebrow. Discharged to full duty with no restrictions and no prescriptions.
Employee fell while entering a skidsteer. EE lost balance and fell with arms stretched out. X-rays confirmed EE had broken EE's elbows and physical therapy was prescribed on 06/17/2024. At first, PT was prescribed for degenerative non-work-related issues with shoulder, but after investigation, the doctor stated PT was needed for elbows. Might be over reporting here.
The employee was helping unclog a shoot non belt 8. After this was completed the employee stepped onto the walkway. The panel of the walkway gave way and the employee fell to the ground.
Employee was getting off dozer when they slipped off the tracks. The tracks were full of red clay. When they slipped the left leg buckled and fell on the track of the dozer. Pulled muscle on left leg.
This started out as a non reportable but due to complications it had turned into a reportable. The employee was helping unload some cross ties off a flatbed truck. When he cut the band around the cross ties they sprang forward pushed the employee off the truck to the ground. (3feet). Again this was a non reportable but due to complications it has turned into a reportable.
EE was preparing to check and or fine tune adjustments on a classifier rising current valving. When EE stepped on scaffold planking to perform the above procedure, the planking broke causing EE to fall. The fall injured EE's back, right wrist and right hand.
EE had just parked loader; started down steps of loader when his left foot got hung in the ladder. EE fell backward; regained his balance and landed on his feet with all of his weight on the left side and left ankle buckled.
