EE was preparing to check and or fine tune adjustments on a classifier rising current valving. When EE stepped on scaffold planking to perform the above procedure, the planking broke causing EE to fall. The fall injured EE's back, right wrist and right hand.

He was lifting pipe when forklift turned over.

Running from a flyrock, tripped on gravel, fell, hurt left leg & left arm
