Showing all 27While unloading a mounted 26.5R25 Tire from service vehicle to spare rack, tire & wheel assembly hit the spare rack and came off Bead Hook being used to lift assembly. It fell to ground pushing injured into side of service truck. Resulting in contusion to posterior portion of left shoulder/rib area and fractured ribs.
Employee was walking down a slope of a mountain and tripped over a piece of sagebrush causing him to trip. Resulting in a sprained left ankle.
On 2/15/2007 an employee reported an incident occurring back in August of 2006 where he hyper-extended his knee and then two more incidents at work and one at home. Employee continued to have problems with knee and eventually had surgery to repair a ligament. Employee lost 4 days and had 9 restricted days.
Driller slipped and caught his (r) ankle in gap on deck. (r) ankle sprain.
The EE was in a hurry and was stepping back into his six yard loader he was operating. His muddy boot slipped off the step and he hit his knee on the edge of the cab.

THE EMPLOYEE HAD JUST FINISHED CLEANING THE CONVEYOR BELT & STARTED TO STEP BACKDOWN TO CATWALK WHEN HE LOST HIS BALANCE AND TWISTED HIS ANKLE. HE WAS TAKEN TO THE DOCTOR AND PLACED ON RESTRICTED DUTY.
Employee was assisting another miner to unload prell pot out of the back of a pick up. The employee was getting out of the back of the truck, squated down and lost his balance. Employee tried to get his balance back by putting his foot out in the process twisted his foot wrong.
Employee was cleaning up oil pads from under a 4 yard LHD, foot slipped when he was standing up - causing him to twist back and catching himself.
Employee was helping to move some old core boxes off of the top shelf, he took a step back & lost his balance. His right foot rolled underneath his right ankle. He returned to work the following day & the client sent him home because they did not have light duty work for him. Client said that he would need a doctor's release for full duty. Drug test results: Negative.
EMPLOYER WAS A DRILL HELPER ON OUR U8 CORE RIG. THEY WERE LOOKING FOR THE GRINDER & EMPLOYEE STEPPED OFF OF THE BACK OF THE MAN BASKET ON THE SKY TRACKER. WHEN HE DID, HE CAUGHT HIS CLOTHING ON METAL CAUSING A LACERATION.
Employee turned off air and water valve and stepped back onto roadway and caught a rock with the heel of his boot, slipped and twisted his right ankle.

Ee was climbing off L002 mucker when his right foot slipped. His left hand was still hanging on to the mucker when his foot hit the ground. He felt a snap and wasn't able to pick up anything. He was evaluated by orthopedic on 8/5/04 and injury will require surgery to re-attach bicep tendon.
Employee stepped from water truck to drill rig cat walk. He lost his balance and grabbed suspended piece of casing to regain balance. The casing swung into side of drill derrick pinching his left hand between derrick and casing.
THE MINER DROPPED THE DRILL AND LEG, AS HE PICKED UP THE DRILL HE LOST HIS FOOTING AND FELL INTO THE MACHINE. THE CONTACT OF HIS HEAD WITH THE MACHINE RAISED 2 KNOTS OR CONTUSIONS AND A .5 INCH LONG BREAK IN THE SKIN. THE CUT ON HIS FOREHEAD REQUIRED 2 STITCHES.
THE EMPLOYEE WAS WALKING ALONG A BUNDLE OF GROUND SUPPORT WIRE WHEN THE BUNDLE FELL AGAINST HIM. AS IT FELL HE TWISTED HIS ANKLE TRYING TO GET AWAY FROM IT.
LOST FOOTING AND PUT ALL HIS BODY WEIGHT ON WRIST WHILE TIGHTENING CHAIN BINDER.
HE WAS STEPPING OFF DECK ONTO STEP WHEN STEP BROKE IN HALF - HIS LEFT KNEE BUCKLED AND WENT TO THE GROUND.
WHILE WALKING IN SHOP, EMPLOYEE STATES HE STEPPED INTO OPEN LUBE PIT.
