The employee had just finished the post-trip inspection of equipment they operated at the Santa Margarita Quarry. The employee wanted to check out the new frontend loader, so they started to walk around the and look at the new equipment. The new loader was backed into a cutout to prevent the equipment from moving and the employee stepped into the cutout and slipped and fell.

Employee was cutting metal with a torch which was supported on sawhorses. The employee moved one sawhorse and continued to use the torch to cut the metal. The employee turned to make a step and tripped over the sawhorse that they had moved earlier. The employee fell and landed on their right shoulder.

Temporary scaffolding staircase was removed. Area was not safely cordoned off correctly. Employee fell from 7' elevated height at doorway with no stairs. Employee suffered bruising and laceration.

An employee was walking up the shop stairsteps when they inadvertently slipped. The employee did not fall to the ground after holding on to the steps. They felt a pain in the left toe, which was diagnosed as a closed fracture.

Walking on an uneven surface while performing a pre-task inspection, left knee hyper-extended as he strode and hit a depression.

Employee was climbing down into the feeder that was being installed into #2 conveyor tunnel. The 4' "A" frame ladder slipped from under him and he hit his right elbow and right shoulder. His left knee was reported as sore due to walking on even surfaces during this project.

The employee was dismounting a CAT 980F front end loader on level ground with engine off and parking break set. It was daylight the weather clear dry. While turning to face the ladder to dismount the employee twisted his right knee. The injury was diagnosed as a mild sprain. The injury became a time lost accident on 10/8/10 when surgery was required for a torn meniscus.
