Showing all 10Heel of left boot was cut off by employee while carrying 2" air pump. He slipped on grounding mat at DC power center - mat was damp and slippery. He twisted his left knee and was slightly swollen.
Installing cover on cutter head. Slid/lost footing and hit his head. Briefly unconscious/dizzy. Laceration on elbow.

Injured fell on his leg while moving a shuttle car cable. He had swelling of his ankle and went to emergency room. He was diagnosed with pulled ligament and did not miss work.
Chute feeding the scalping screen had plugged causing a few rocks to fall on floor below. The individual had cleared the chute and was going back to bottom floor of structure. The laborer did not have time to get to spill at this time. The individual stepped on one of the rocks causing him to fall onto the floor grating.
