Showing all 15Mechanic was climbing onto a Cat D11 Dozer when his foot slipped off of the foot step and he fell landing on his right foot slanted inward.
Fueler was removing fueling hose on a 992 loader when he felt a sharp pain in his left calf and felt pressure on it. He fell to the ground for a minute and then continued to work.
Operator was operating a loader on the upper stockton coal pit, when he became light headed and hot. The operator stopped the loader and stepped out onto the cat walk to get some fresh air. At this time he briefly lost consciousness and fell, striking his lip and cheek on the railing. Operator was taken to Logan Hospital for Dr. evaluation.
Employee slipped off track of dozer while changing fuel filters, jarring lower back.
THE MAINTENANCE EMPLOYEE ATTEMPTED TO NOTIFY THE LOADER OPERATOR OF HIS APPROACH TO PERFORM MAINTENANCE ON THE LOADER. THE OPERATOR DID NOT HEAR HIM AND WHILE MAINTENANCE EMPLOYEE WAS CLIMBING UP THE LADDER OF LOADER, THE LOADER OPERATOR PULLED OUT CAUSING HIM TO LOSE HIS GRIP.
Climbing off of left side of 992G Loader and fell from steps.
Employee was stepping down from grader. He said his right hand gave away and let him fall onto the corner of blade and circle of the grader.

EE was filling up fuel truck. He climbed up ladder to see if truck was full. He was climbing down and he slipped on ladder and fell on ground. He said his right shoulder and neck were hurting. Sent him to hospital (A.R.H.)
